摘要
目的观察30%脂肪乳剂对异氟醚的血/气分配系数(B/G)的影响以及静脉注射不同剂量的30%脂肪乳剂对异氟醚"洗入"的影响。方法(1)采集桡动脉血,配成不同浓度的30%脂肪乳剂与血的混合液。用二次平衡法测定异氟醚对上述混和液的B/G。(2)24例行冠状动脉旁路移植术的患者分为四组,每组6例。麻醉诱导后,静脉单次注射0、0.2、0.4及0.8ml/kg的30%脂肪乳,随后开启异氟醚挥发罐,将挥发罐刻度设定在2%,并同时用微量泵注入相应剂量的30%脂肪乳0、0.2、0.4、0.8ml·kg-1.h-1。持续吸入异氟醚1h,监测异氟醚呼气末浓度(Fa)和吸入浓度(Fi)。其中0ml/kg的30%脂肪乳组为对照组。结果(1)异氟醚B/G随血中混合的30%脂肪乳剂浓度的增加而升高。(2)0.8ml/kg剂量组的异氟醚Fa/Fi值与对照组比较明显降低(P<0.05)。结论注入较大剂量脂肪乳剂后,造成异氟醚"洗入"速度减慢,这可能是由于输注的脂肪乳剂造成异氟醚的B/G增大所致。
Objective To study the effect of 30% intralipid on the blood/gas partition coefficients（B/G） and uptake of isoflurane. Methods A mixture of blood and 30% intralipid in different concentration was made. B/G of isoflurane was measured by syringe-syringe double equilibrium method. Twenty-four patients undergoing selective coronary artery bypass were randomly divided into 4 groups. After induction,the isoflurane vapour was set at 2% ,and a bolus of different dosage（0,0.2, 0.4 or 0.8 ml/kg） of 30% intralipid was injected intravenously, followed by continuous infusion of intralipid in a dose of 0,0.2,0.4 or 0.8 ml·kg^-1·h^-1 for one hour. The end tidal concentration（Fa） and inspired concentration（Fi） of isoflurane were recorded. Results （1） As the concentration of intralipid in the blood was increasing,the B/G of isoflurane was increasing. （2）The ratio of Fa/Fi in group Ⅳ was significantly lower than that in control group. Conclusion Infusion of large dose of intralipid may decrease the rising in the ratio of Fa/Fi by increasing the B/G of isoflurane.
