摘要
为克服鼻胆管引流(ENBD)引流管较细易堵塞、易脱落、胆汁外引流过多易致胆汁丢失综合症等缺点,作者根据胆总管解剖和重症急性胆管炎(ACST)梗阻发病特点,将引流管前端14cm改进使其进入胆总管形成“O”形而被支撑于乳头和胆总管下端。观察30例患者普通和改良ENBD不同方法的效果,结果表明改良ENBD胆汁外引流量比普通ENBD减少50%以上(P<0.01);脱管率由普通ENBD的27%降至改良ENBD无脱管(P<0.05)。提示改良ENBD可减少胆汁丢失,减少引流管阻塞可能,防止导管脱落。
The purpose of this study was to overcome the defect of endoscopic nasobilliary drainge (ENBD) catheter obstruction and delocation due to its thinner culibre as well as massive loss of bile.According to anatomy of common bile duct and clinical feature of acute cholangitis of severe type(ACST),the distal 14cm end of the catheter was modified to let it form shape “O” after entering the common bile duct , then the catheter was supported at distal end of the common bile duct and duodenal papilla . A consecutive series of 30 patients with ACST were treated with common ENBD and modified ENBD respectively,and the results of treatment compared.It was showed that bile loss treated by modified ENBD decreased more than 50% as compared with that by common ENBD( P <0 01).The rate of catheter delocation from common bile duct was reduced from 27% to zero( P <0 05).It is concluded that modified ENBD can decreas massive loss of bile,prevent catheter obstruction and detachment.
