初诊2型糖尿病患者血浆FGF-21水平改变 被引量:2

Changes of plasma FGF-21 level in the newly diagnosed patients with type 2 diabetic mellitus

摘要 目的:探讨2型糖尿病(T2DM)患者血浆FGF-21水平变化及其与体重指数(BMI)、腰臀比(WHR)、血糖、血浆胰岛素水平等的关系。方法:采用放射免疫法测定了T2DM患者及正常人血浆FGF-21水平,并分析了血浆FGF-21水平与BMI、WHR、胰岛素抵抗指数(HOMA—IR)、胰岛β细胞分泌指数(HOMA—IS)、血脂、血糖、血浆胰岛素和游离脂肪酸(FFA)水平等的关系。结果:T2DM患者血浆FGF-21水平明显高于对照组(1.82±0.64VS1.53±0.60ng/L,P〈0.05)。血浆FGF-21水平与FFA、HOMA—IS呈明显正相关(r=0.36,r=0.33,均P〈0.05),与WHR、空腹血糖(FBG)、负荷后2h血糖(2hPBG)呈明显负相关(r=-0.23,r=-0.39,r=-0.28,均P〈0.05)。多元回归分析表明腰臀比(WHR)是影响血浆FGF-21水平的独立相关因素。结论:血浆FGF-21水平的改变与代谢紊乱有关.并可能参与了T2DM的发生和发展。 Objective:To investigate the changes of plasma FGF-21 levels and the relationship between plasma FGF-21 levels and body mass index(BMI), waist hip ratio(WHR), blood glucose, plasma insulin levels as well as other factors in newly diagnosed type 2 Diabetic mellitus (T2DM). Methods:Plasma FGF-21 levels were assayed by ILIA in the patients with T2DM and in the controls. The relationship between plasma FGF-21 levels and BMI, HOMA-IR, HOMA-IS, blood lipids, plasma glucose, insulin, free fatty acids (FFA) levels were also analyzed. Results:Plasma FGF-21 levels in patients with T2DM were significantly increased compared with controls (1.82± 0.64 vs 1.53±0.60 ng/L,P 〈 0.05). Plasma FGF-21 levels correlated positively with FFA, HOMA-IS (r=0.36, r=0.33, all P〈0.05), and negatively with WHR, FBG and 2hPBG (r=-0.232, r=-0.385, r=-0.276, all P〈 0.05). Multiple regression analysis showed that WHR was independent related factor influencing plasma FGF-21 levels. Conclusion :The changes of FGF-21 levels are associated with metabolism disorder, and it may contribute, partly to the pathogenesis of T2DM.
