DNA倍体分析和癌胚抗原检测在恶性胸腔积液诊断中的价值 被引量:3

摘要 目的通过流式细胞仪对确诊为恶性胸腔积液患者的胸水脱落细胞进行DNA倍体分析,并同肿瘤标记物癌胚抗原(CEA)检测方法相比较,以评估两种方法及其联合检测在诊断恶性胸腔积液中的价值。方法选取经过临床确诊的38例恶性胸腔积液患者为试验组,40例良性胸腔积液患者为对照组。使用流式细胞仪对胸水脱落细胞进行DNA倍体分析,并同时检测胸水的癌胚抗原。结果DNA倍体分析的灵敏度为84.21%,特异度为80%,约登指数(Youden指数)为0.64,准确度为82.05%。CEA检测灵敏度为60.53%,特异度为97.5%,Youden指数为0.58,准确度为79.49%。两种方法诊断试验灵敏度和特异度比较差异均有显著性(P<0.05,P<0.01),但Youden指数和准确度比较差异无显著性(P>0.05)。两者联合诊断的灵敏度为93.77%,特异度为78%,Youden指数为0.72。结论使用流式细胞仪DNA倍体分析检测胸腔积液具有较高的灵敏度和特异度。如果联合CEA检测,对恶性胸腔积液有更好的诊断价值。 Objective To assess the diagnostic values of DNA anlysis by Flow Cytometry and CEA in malignant pleural effusions. Methods Thirty-eight patients with malignant pleural effusions were diagnosed with clinical methods formed the tumor group, while other 40 patients with benign effusion formed the control group, All the patients' pleural effusion was detected by the DNA heteroploid detection with flow cytometry and CEA detection. Results The sensitivity of the DNA heteroploid detection with flow cytometry was 84. 21%, the specificity was 80%, the Youdeffs index was 0.64 and the accuracy was 82.05%. The sensitivity of the CEA detection was 60.53% ,the specificity was 97.5%, the Youden's index was 0.58 and the accuracy was 79.49%. There was significant difference in the sensitivity and specificity between the two detections(P〈 0.05,P〈0.01) ,but there was not significant difference in the accuracy and Youden's index(P〉 0.05). The sensitivity of the combination of the two detections was 93.77% ,the specificity was 78 % and the Youden's index was 0.72. Conclusion DNA anlysis using Flow Cytometry can be simplily and rapidly performed and is highly sensitive and specific. There is better diagnostic values for malignant pleural effusions if combined the CEA detection.
