摘要
目的:探讨HBV感染时的肝细胞凋亡及其参与的病理机制。方法:研究HBV相关CH38例、LC27例和HCC43例的肝组织,以抗Fas单抗检测Fas表达及以标记的核苷酸借3'未端转移酶检测DNA损伤。结果:Fas表达CH94.7%、LC59.3%、HCC41.9%,与炎症重度明显相关。DNA损伤也与炎症一致。原位展现的凋亡细胞,主要在碎屑样坏死区。结论:CTL-Fas-凋亡也是参与肝细胞损伤的重要机制。在HCC,Fas还与突变型p53的表达相关,p53基因突变使细胞生长失控而癌变;细胞生长失控可能也是激发Fas表达的信号。
Aim: The aim was to clarify the role between hepatocyte Fas expression and apoptosis in hepatitis B virus related chronic liver diseases. Methods: The liver tissues were investigated by in situ immunohistochemistry and TUNEL test. ResuIts: Fas expression in hepatcyte was found in 94.7% of 38 cases with chronic hepatitis. 59.3% of 27 with liver cirrhosis, and 41.9% of 43 with hepatocellular carci- noma respectively, significantly correlated with the severity of inflammatory lesions. The nuclei stained witl. TUNEL showed the 3'-OH end of DNA which marked impaired DNA, and also correlated positively with imflammation lesiocts. Apoptotic cells distributed mostly in the piecemeal necrotic region. ConcIusion: The CTL-Fas-apoptosis system was also involved in the mechanism of liver damage of hepatitis B. In hepatocellular carcinoma there was a relationship between the expression of Fas and p53 mutant.
