摘要
目的：研究糖尿病（DM）合并高血压（HP）患者下肢动脉粥样硬化（AS）的发病率，重视DM合并HP患者AS的防治，为临床提供理论依据。方法：应用高频彩色多普勒超声观察120例DM合并HP患者（A组）与98例单纯DM患者对照组（B组）下肢动脉。对两组下肢动脉内-中膜厚度、有无粥样硬化斑块的发生、管径狭窄或闭塞及血流动力学改变进行比较。结果：A组下肢血管管腔的内-中膜厚度、硬化斑块、狭窄、闭塞的发生率明显高于B组。内-中膜厚度增加及硬化斑块的发生率以股动脉明显，而管腔狭窄、闭塞以胫前动脉为重，足背动脉次之。结论：DM合并HP患者较单纯DM患者更容易并发和加重下肢AS，高频彩色多普勒超声检查可以为临床诊疗提供可靠的信息，有利于临床医生对病情作出正确判断及采取相应治疗措施。
Objective To investigate the incidence rate of lower limb atherosclerosis in diabetic patients complicated by hypertension, attach importance to the prevention and treatment of this disease, and provide theoretical evidence for clinical applications. Methods Cases were 120 diabetic patients complieated by hypertension （group A）. Controls were 98 diabetic patients （group B）. The artery of the lower limb was examined by high-frequency color Doppler ultrasonography. The incidence rates of intima-media thickness increase, atherosclerotic plaques, lumen, occlusion, and changes in hemodynamics were compared between cases and controls. Results The incidence rates of intima-media thickness increase, atherosclerotic plaques, lumen, and occlusion in the lower limb artery of the group A were significantly higher than those of the group B （ （ P 〈 0.05 ）. Intima-media thickness increase and atherosclerotic plaques occurred mainly in the femoral artery, whereas lumen and occlusion mostly occurred in the anterior tibial artery, followed by the foot arch artery. Conclusion Diabetic patients complicated by hypertension were more inclined to complicate and aggravate lower limb atherosclerosis than those without hypertension. The high-frequency color Doppler ultrasonography could provide reliable information for clinical diagnostics and treatment and is beneficial to physicians in correct diagnostics and related treatment.
