摘要
目的研究肝中静脉属支与胆囊床的关系。方法解剖观测137例成人肝脏标本中突入胆囊床的肝中静脉属支直径、长度、走行及距胆囊床较近肝中静脉的属支与胆囊壁的最短距离。结果突入胆囊床肝中静脉属支与胆囊壁的最短距离为(0.26±0.20)cm;突入胆囊床肝中静脉属支直径、长度分别为(0.31±0.04)cm、(2.85±1.55)cm。突入胆囊床的肝中静脉属支走行方向是由右前下向左后上。结论(1)突入胆囊床的肝中静脉属支破裂是引起胆囊切除时胆囊床大出血的重要原因;(2)在胆囊床由左前下向右后上方缝合肝组织是对突入胆囊床肝中静脉属支破裂出血的有效预防方法。
Objective To study applied anatomy of relationship of branches of venae hepaticae intermediae and beds of gallbladders. Methods The diameter,length,courses of branches of venae hepaticae intermediae intruding beds of gallbladders,and the shortest distance between branches of venae hepaticae intermediae and beds of gallbladders were observed in 137 liver specimens of adults. Results The shortest distance between branches of venae hepaticae intermediae and bed of gallbladder,the diameter,length of the branches of venae hepaticae intermediae intruding beds of gallbladders were (0.26 ± 0.20) cm, (0.31 ± 0.04) cm and (2.85 ± 1.55 ) cm respectively. The courses direction of the branches of venae hepaticae intermediae intruding beds of gallbladders was from right - ante - infer to left - post - supra. Conclusion The rupture of the branches of venae hepaticae intermediae intruding beds of gallbladders is the important reason causing the bleeding on beds of gallbladders during cholecystectomize. It is an effective way suturing hepatic tissue on vertical direction from right - ante - infer to left - post - supra using bigger orthodrome needle.
