腕瘘失功能后在邻瘘口处行瘘成形术的临床研究 被引量:1

摘要 目的:总结维持性血液透析(MHD)患者腕瘘失功能后在邻瘘口处行瘘成形术的临床资料,为节约外周静脉提供一可行方法。方法:23例腕瘘失功能患者(A组),在邻瘘口处利用已扩张的头静脉与桡动脉再次行内瘘成形术;同期腕部桡动脉-头静脉瘘首次成形手术患者75例作为对照组(B组),比较其手术成功率、动脉及静脉内径、吻合口长度。结果:A组失功能部位82.61%在瘘口处,13.04%为头静脉血栓,其动、静脉内径明显比B组大[分别为(3.46±0.21)mm对(2.62±0.19)mm,(3.51±0.35)mm对(2.43±0.23)mm,P<0.001];A组吻合口大小(4.14±0.49)mm、B组为(7.35±1.63)mm,A组吻合口明显比B组小(P<0.001)。A组手术均成功,B组手术成功率90.67%,但与A组相比差异无显著性(P>0.05)。A组术后1~2周内使用瘘,透析血流量>300ml/min,B组术后(5.19±1.19)周使用瘘。结论:腕瘘失功能的部位主要在瘘口;利用已扩张的头静脉行瘘成形术的成功率高、瘘成熟期短、血流充足,可节约MHD有限的外周静脉。
