摘要
目的探讨腹腔化疗灌注对存在破裂出血的肝癌腹腔转移的预防作用。方法对28例存在肝脏破裂出血的肝癌患者,13例单纯行肝动脉栓塞止血和肝癌的TACE治疗（对照组）,15例行上述治疗联合腹腔化疗灌注（治疗组）。治疗次数及间隔时间相一致。随访记录两组患者的腹腔转移与生存情况。结果两组病例随访3-12个月。对照组发生腹腔转移9例,3、6和12个月腹腔转移率分别为55.6%、100%和100%,其生存率分别为69.2%、30.8%和7.70%。治疗组仅见2例腹腔转移,其中3、6和12个月肺转移率分别为0、7.70%和14.3%,生存率分别为93.3%、86.7%和46.7%。两组腹腔转移率及生存率对比差异有统计学意义。结论对存在破裂出血的肝癌行预防性腹腔化疗灌注可以有效预防腹腔转移,提高病人生存率。
Objective To study the preventive value of belly cavity chemotherapy infuses in metastases of from liver cancer with rupture. Methods Twenty - eight cases of liver cancer with rupture were divided into two groups. In contrast group, 13 cases were treated only by embolization to stop bleeding and transarterial chemoembolization （TACE）. In treatment group ,combined therapy of 15 cases with TACE and. belly cavity chemotherapy infuses was given to prevent metastases of belly cavity from liver cancer with rupture. The cases were followed up from 3 to 12 monthes. The belly cavity metastatic rate and the survival rate of the patients were recorded in two groups. Results In contrast group,the belly cavity metastatic nodules were found in 9 of 13 cases. The 3,6,12 month metastatic rates were 55.6%, 100% and 100% respectively,and the survival rates were 69.2% ,30.8% and 7.70% respectively. In treatment group,the metastases were found in 2 of 15 patients. The 3,6,12 month metastatic rates were 0,7.70% and 14.3 % respectively, and the survival rates were 93.3% ,86.7% and 46.7% respectively. The belly cavity metastatic rates and the survival rates of two cases were apparently different. Conclusion Belly cavity chemotherapy infuses is a very effective method for prevention of the belly cavity metastases from liver cancer with rupture. Combined use of TACE and belly cavity chemotherapy infuses is superior to TACE alone for reducing the metastatic rate of belly cavity and increasing the survival rate for cases of liver cancer with rupture.
