尿脱落细胞端粒酶活性的检测对膀胱肿瘤诊断意义的Meta分析

摘要 ①目的 探讨应用Meta分析判定尿脱落细胞端粒酶活性和膀胱肿瘤的相关性.②方法 采用固定效应和随机效应两种模型,合并效应尺度OR,并汇总尿脱落细胞端粒酶活性检测在膀胱肿瘤诊断中的敏感度和特异度,评价该标志物的效度.③结果 采用随机效应模型:计算OR合并为27.15;采用固定效应模型:计算OR合并为33.87(95%可信区间为25.87~44.36),将同质的23项研究结果加权合并后,膀胱肿瘤组累积为1238例,对照组累积为1136例,检测此标志物敏感度为0.7649,特异度为0.9067,阳性似然比为8.20,阴性似然比为0.26.④结论 尿脱落细胞端粒酶是一种理想的膀胱肿瘤生物学标志物,meta分析结果为将此标志物用于膀胱肿瘤的诊断提供了详实的科学依据. Objective To systematically evaluate the relationship between telomerase activity in urine exfoliated cells and bladder tumors with meta analysis. Methods The sensitivity, specificity and validity of telomerase activity in urine exfoliated cells were evaluated by fixed effect model, random effect model and combined odds ratio(OR) in bladder tumors diagnosis. Results The combined OR was 27.15(95% CI:17.97 -41.03) and 33.87(95% CI:25.87 -44.36) respectively by random effect model and fixed effect model. Bladder tumors group and control group was 1238 cases and 1136 cases respectively after 23 studies combined, The sensitivity, specificity , positive likelihood ratio and negative likelihood ratio of telomerase activity in urine exfoliated cells was 0. 7649,0.9067,8.20 and 0.26 respectively. Conclusion Telomerase activity in urine exfoliated cells was ideal biological marker of bladder tumors. The results would provide theoretical basis for bladder tumors diagnosis.
