透明质酸钠不同给药途径干预早期骨关节炎兔软骨细胞的凋亡 被引量:7

Effect of sodium hyaluronate via different administration routes on chondrocyte apoptosis in rabbits with early osteoarthritis

摘要 目的:观察前交叉韧带横断造成兔早期骨关节炎软骨细胞凋亡的变化,以及应用关节内和静脉注射透明质酸钠对软骨细胞凋亡的影响。方法:实验于2006-04/09在华中科技大学同济医学院协和医院骨科实验室完成。共选取36只新西兰兔,随机数字表法分成正常组、骨关节炎组、关节内注射透明质酸钠组、静脉注射透明质酸钠组,每组9只。正常组不做任何处理,作为对照;骨关节炎组仅行前交叉韧带横断术;关节内注射透明质酸钠组行前交叉韧带横断术后给予关节腔内注射透明质酸钠;静脉注射透明质酸钠组行前交叉韧带横断术后给予静脉内注射透明质酸钠。所有实验动物在术后8周处死。应用苏木精-伊红染色观察关节软骨组织的病理学变化,原位末端标记法标记凋亡细胞,在前交叉韧带横断术前、术后6,8周分别进行血液检测。观察各组动物的组织学检查、Mankin评分、凋亡指数和血液检测结果。结果:纳入新西兰兔36只,均进入结果分析。①所有动物的血液检查包括红细胞、白细胞、血小板计数以及血尿素氮、肌酐、电解质,结果均在正常范围内。②应用透明质酸钠干预的关节内注射透明质酸钠组和静脉注射透明质酸钠组与未用透明质酸钠干预的骨关节炎组相比较,前者组织病理学改变明显轻于后者。③骨关节炎组的Mankin评分与其他三组差异有显著性[(8.97±1.02),(0.00±0.00),(5.62±0.86),(6.13±0.64)分,P<0.05]。④各组均发现软骨细胞凋亡,并且骨关节炎组的凋亡指数显著高于其他三组[(27.38±3.46)%,(5.17±1.06)%,(12.47±2.13)%,(14.51±2.87)%,P<0.05],正常组的凋亡指数与其他三组差异有显著性意义(P<0.05)。结论:前交叉韧带横断所致兔早期骨关节炎的软骨细胞凋亡明显增加,关节内和静脉内注射透明质酸钠均能有效抑制其软骨细胞凋亡。 AIM: To investigate the changes in apoptotic chondrocytes in rabbits with early osteoarthritis induced by anterior cruciate ligament transaction, and evaluate the suppressive effect of intraarticular and intravenous injection of sodium hyaluronate (SH) on the progress of chondrocytes apoptosis. METHODS: The experiment was conducted at the Laboratory of Orthopaedics Department, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology from April to September 2006. Thirty-six New Zealand rabbits were randomly divided into normal group, osteoarthritis group, lASH group and IVSH group with 9 rabbits in each group. Rabbits in the normal group received no treatment and served as control; the osteoarthritis group underwent antenor cruciate ligament transaction only; Rabbits in the lASH group received intraarticular injection of SH after anterior cruciate ligament transaction; the IVSH group received intravenous injection of SH after anterior cruciate ligament transaction. All rabbits were sacrificed at 8 weeks after surgery. Pathological changes of articular cartilage tissue were observed with haematine-eosin staining, and apoptosis was labeled with TUNEL. Blood examinations were performed before and 6 and 8 weeks after anterior cruciate ligament transaction. Histological test, Mankin score, apoptosis index and blood examinations of each group were observed. RESULTS: All 36 rabbits were involved in the result analysis. (1)Blood examinations of all rabbits including red blood cells, white blood cells, and platelets count, blood urea nitrogen, creatinine and electrolytes were within the normal ranges. (2)Histological changes observed in IASH group and IVSH group were obviously slighter than those in the osteoarthritis group. (3)Differences of Mankin score between osteoarthritis group and the other 3 groups were significant [(8.97±1.02), (0.00±0.00), (5.62±0.86), (6.13±0.64) points, P 〈 0.05]. (4)Apoptotic chondrocytes were observed in all groups, and the apoptosis index of the osteoarthritis group was significant higher than the other groups [(27.38±3.46)%, (5.17±1.06)%, (12.47±2.13)%, (14.51±2.87)%, P 〈 0.05], and there were significant differences between the normal group and the other groups (P 〈 0.05). CONCLUSION: Chondrocyte apoptosis is increased significantly in rabbits via early osteoarthritis induced by anterior cruciate ligament transaction, lASH and IVSH could inhibit the chondrocyte apoptosis effectively.
