摘要
目的：分析胰腺腺鳞癌的临床和病理学特点,提高对该病的认识.方法：回顾性分析5例胰腺腺鳞癌患者的临床资料.结果：本组患者男4例,女1例,平均年龄54.2岁;以腹痛、消瘦为主要症状;肿瘤位于胰头颈部2例,体尾部2例,胰尾部1例;行胰头十二指肠切除术1例,胰体尾部加脾切除1例,姑息性手术2例,1例未行手术,B超引导下穿刺活检确诊;肝转移2例,腹腔广泛转移1例;病理结果：镜下表现为腺癌和鳞癌细胞混杂;随访4例,术后生存4～15月,死于复发和转移.结论：胰腺腺鳞癌是临床上罕见的一种恶性肿瘤,腺癌和鳞癌混杂为组织学特点,呈侵袭性生长,易局部侵犯和远处转移,预后差.
Objective： To analyze the clinical and pathologic characteristics of pancreatic adenosquamous carcinoma . Methods ： We retrospectively studied the clinical datas of 5 cases of adenosquamous carcinoma of the pancreas hospitalized from 1992 to 2004 and reviewed the literature. Results：The series was composed of 4 men and 1 women with a mean age of 54.2 years. The cardinal symptoms were abdominal pain and emaciation. The turners were located in the pancreatic head of 2 cases, in the pancreatic body and tail of 2 cases,in the pancreatic tail of 1 case. 1 case was performed with pancreatoduedenectomy, 1 case with distal pancreatectomy with resection of the spleen,2 cases with palliative surgery, 1 case with ultrasonically guided fine needle aspiration biopsy. The series included 2 cases with hepatic metastasis, 1 case with abdominal extensive metastasis. The micrography was characterized by an admixture of adenomatous and squamous cell elements. 4 cases had their follow - up. They survived from 1 month to 15 months after operation, and died of recurrence and metabasis. Conclusions ：The pancreatic adenosquamous carcinoma is a rare malignant tumor with an admixture of adenomatous and squamous cell elements in microanatomy. It often shows aggressive biological behavior, invasion to adjoined organs and metastasis. The prognosis of the disease is very poor.
