摘要
目的 探讨超声造影评价胶质瘤血供及微波消融疗效的应用价值。方法 建立20只Wistar大鼠皮下胶质瘤模型,随机分为治疗组(10只)、对照组(10只),治疗组给予微波消融(30W、40s),对照组仅置入微波天线。术前20只均行常规超声及超声造影检查;治疗组术后以能量多普勒超声及超声造影评价疗效,并与病理结果对照。结果 造影后能量多普勒超声检测胶质瘤血流分级明显高于造影前(P〈0.05)。实时超声造影可清晰显示胶质瘤的滋养血管,动态观察微循环灌注及定量分析血流灌注参数。微波消融术后能量多普勒超声检查9只未见血流信号,超声造影显示8只部分增强,病理证实10只均消融不全,超声造影与病理符合率显著高于能量多普勒超声(P〈O.05)。结论 造影后能量多普勒超声检测低速血流优于造影前。实时超声造影能动态显示胶质瘤的微循环灌注及准确评价微波消融术后近期疗效。
Objective To investigate the application value of contrast-enhanced ultrasonography (CEUS) for evaluation blood supply and microwave ablation effect of glioma. Methods C6 subcutaneous glioma models of 20 Wistar rats were established,and were distributed randomly into treatment group(10 cases) and contrast group(10 cases). Gliomas in treatment group were treated with microwave ablation at 30W for 40s and those in contrast group received sham operation. All cases were detected by baseline ultrasonography and CEUS before microwave ablation. Therapeutic effect were evaluated with power Doppler imaging(PDI) and CEUS after treatment, compared with pathology. Results The blood flow classification in 20 cases assessed with PDI after injection SonoVue were higher than that before injection, the difference between them had statistical significance ( P 〈 0.051. With real time CEUS, nourishing vessels were clearly displayed in 20 gliomas, and microcirculation perfusion were observed dynamically. Eighteen of them represented homogeneous enhancement. Time to enhancement and time to peaking intensity were (1 0.90 ± 2.84) s, (37.64 ± 12. 15) s, respectively. Peaking intensity was ( 15.62 ± 8.11 ) dB. Enhancement duration was (271.18 ± 42.17)s. After microwave ablation,8 cases were partial enhancement with real time CEUS, while there were no blood flow signals in 9 cases with PDI,the coincidental rate of PDI and real time CEUS with pathological results were 10 ~,80~ ,respectively ( P d0.05). Conclusions PDI after injection SonoVue is more sensitive for low velocity blood flow than before injection. Real time CEUS could reveal microcirculation perfusion of glioma dynamically, and evaluate the short-term effect of microwave ablation accurately.
