摘要
目的探讨腔内治疗联合股浅静脉瓣膜缩窄术治疗下肢慢性静脉溃疡的疗效。方法26例下肢慢性静脉溃疡患者证实合并髂静脉狭窄和深静脉瓣膜返流,先经腔内治疗髂静脉狭窄,一周后行大隐静脉高位结扎剥脱术加交通支结扎同时行股浅静脉瓣膜缩窄术,溃疡予以清创换药。以CEAP临床分类与临床记分和VRT试验评价疗效。结果置入支架后造影见髂静脉恢复通畅,支架到位及展开均良好。术后溃疡于10d～2月均已愈合,随访3～28月,未再有溃疡复发,临床症状缓解,瓣膜功能得到了一定程度恢复。结论腔内治疗联合股浅静脉瓣膜缩窄术对下肢慢性静脉溃疡有较好的疗效。
Objective To evaluate the effect of endovascular stenting combined with external constriction valvuloplasty of superficial femoral vein for the treatment of chronic venous ulcer. Methods Thirty-two limbs in 26 patients who were proved to be deep venous insufficiency （DVI）and iliac vein stricture,were treated with iliac vein stent implantation or venoplasty,combined with the encircling constriction of venous wall at the first valve of superficial femoral veins.They were also treated with the high ligation and ablation of great saphenous vein,ablation of superficial veins and ligation of perforator veins at the same time. VRT test and CEAP classification and clinical scoring were proceeded from three months to twenty-eight months after operation to evaluate the effect of the operation.Results The iliac veins were all patent after stent implantation.A follow-up of 3-28months, all ulcers were healed without occurrence, the clinical symptoms were eliminated.The function of the valve of superficial femoral veins was recovered.Conclusions Endovascular stenting combined with external constriction valvuloplasty of superficial femoral vein for the treatment of chronic venous ulcer were effective.
