产足月低体重儿产妇胎盘血管内皮生长因子的表达及意义

Expression and role of placental vascular endothelial growth factor in placentas from puerperants with low-birth weight infants

摘要 目的 观察血管内皮生长因子(VEGF)在产足月低体重儿产妇胎盘中的表达及其与胎盘血管网络构建、胎盘重量及新生儿体重的关系。方法 采用免疫组织化学和Western blot法,检测19例正常妊娠胎盘(对照组)和19例产足月低体重儿产妇胎盘(低体重儿组)中VEGF的水平。采用免疫组织化学方法,用抗F8因子抗体标记两组产妇胎盘中的血管密度。两组产妇分娩后,测量胎盘重量及新生儿体重。结果 (1)VEGF在正常妊娠胎盘和足月低体重儿产妇胎盘中分布基本一致,主要分布在滋养层细胞、血管内皮细胞及绒毛间质细胞。在低体重儿组VEGF轻度表达占78.95%,无重度表达;而在对照组VEGF轻度表达只占5.26%,中度表达为52.63%,重度表达为42.11%。低体重儿组中VEGF表达明显低于对照组(P〈0.01)。Western blot结果显示,低体重儿组和对照组胎盘VEGF积分吸光度值分别为39.87±9.07、56.11±15.26,两组相比,差异具有统计学意义(P〈0.01)。(2)低体重儿组胎盘血管密度为(51±9)个/400×视野,低于对照组胎盘血管密度(63±12)个/400×视野(P〈0.05)。(3)低体重儿组与健康对照组的胎盘重量分别为(346.30±61.12)g和(451.40±49.66)g,两组相比,差异具有统计学意义(P〈0.01)。(4)对照组中,胎盘VEGF的表达与血管密度、胎盘重量和新生儿体重的相关系数分别为0.816、0.753、0.896;低体重儿组分别为0.901、0.865、0.812,两者均呈显著正相关(P〈0.01)。结论 产足月低体重儿产妇胎盘中VEGF的低表达可能与胎盘血管生成减少、血流灌注不足有关,在足月低体重儿患者发病中具有一定的意义。 Objective To investigate the expression of placental vascular endothelial growth factor (VEGF) and its relationship with construction of vascular network, placental weight and the newborns' weight in puerperants with low-birth weight infants. Methods The expression of VEGF in 19 placentas was detected by immunohistochemistry and Western blot from normal puerperants (control group) and 19 placentas from puerperants with low-birth weight infants (low-birth weight group). The placental vascular densities were determined by immunohistochemical F8 staining. The placental weight and the newborns' weight were measured after delivery. Results The distribution of VEGF in placenta of normal group was similar to that of low-birth weight group. Most of them were found in villous trophoblast, villous blood vessels and villous stromal cells. VEGF was low expressed mostly in placenta in low-birth weight group (about 78. 95%) and there was no high expression. In control group the VEGF low expression rate was only 5.26%, the moderate was 52.63% and the high was 42. 11%. The VEGF level was significantly lower in low-birth weight group than that in control group (P〈0.01). The placental vascular densities in low-birth weight group and control group were (51±9) and (63±12) counts/400 × vision respectively, and a significant difference was found between them. Placental weight also decreased and showed significant difference in low-birth weight group [(2015±137)g] as compared with control group [(3231 ±263 )g]. In control group, placental vascular density, placental weight and the newborns' weight all had a significantly positive correlation with VEGF(P〈0. 01; r=0. 816, 0. 753, 0. 896 respectively) and in low-birth weight group, the situation was similar (P〈0.01 ; r=0. 901, 0. 865, 0. 812 respectively). Conclusion The decrease of VEGF in placenta from puerperants with low-birth weight infants may be related to inadequate blood vessel formation and may play a role in the generation of low-birth weight infants.
