摘要
目的观察血管内皮生长因子(VEGF)对大鼠缺血皮瓣存活能力的影响。探讨其作用机制和局部更有效的给药途径。方法将45只SD大鼠,随机分为A、B、C三组,每组15只。于背部作6 cm×1.5 cm蒂位于双侧髂棘连线上的全层缺血皮瓣。A组在皮瓣下注射生理盐水,B组皮瓣下注射VEGF生理盐水溶液,C组皮瓣下放置含VEGF生理盐水溶液明胶海绵,原位缝合皮瓣。术后第3,7,14 d观察缺血皮瓣的成活面积,并进行组织学观察,用RT-PCR方法检测皮瓣中VEGFmRNA的含量。结果14天时,C组皮瓣的成活面积率明显高于B组(P<0.05),且B组高于A组(P<0.05)。B组VEGFmRNA表达水平高于A组但低于C组(P<0.01)。结论VEGF的局部应用,可以加速皮瓣内血管再生,从而改善了大鼠缺血皮瓣远端的血供状态,尤其采用明胶海绵携带VEGF皮瓣下放置的方法,更能明显地增强缺血皮瓣的成活能力。
Objective To explore the influence of vascular endothelial cell growth factor (VEGF) on the survival of the ischemic skin flap in rats with the method of RT-PCR,investigate the mechanism and the more effective method of localised delivery of VEGF. Methods 45 SD rats were randomized into three groups. A 6 ×1.5 cm^2 full thickness dorsal ischemic skin flap with the pedicle remaining attached at the interspinous line was elevated on each rat. Saline water was injected under the flap in group A;VEGF solution was injected under the flap in group B;gelatin foam containing VEGF was spreaded under the flap in last group ;Arter that all the flaps were sutured in situs. The area of surviving flap was measured in day 3,Tand 14 respectively. The survive situation of vascular endothelial cell was observed through HE staining The content of VEGFmRNA in the flap was also determined with the method of RTPCR. Results On the 14th day after the treatment, The rate of surviving flap in group C was significantly higher than group B,and the rate of group B was higher than that of group A. Conclusion Localised application of VEGF can ameliorate the microcirculation and accelerate vascularization of the flap, which changes the condition of blood supply in the distal flap and to help the flap survival and also improve the survival rate of the ischemic flap. Gelatin foam with VEGF under the flap would lengthen the action time of VEGF and enhance the survival rate of the ischemic flap more effectively.
