摘要
目的观察自体骨软骨移植治疗膝关节软骨缺损的效果。方法对7例膝关节股骨髁负重部位软骨损伤患者行膝关节镜清理术。摘除关节腔游离软骨碎块2例，半月板部分切除3例，髌上滑膜内侧皱襞切除3例。在股骨内侧髁部取自体骨软骨柱3～5枚，移植到股骨内侧髁软骨损伤部位。结果7例患者手术后伤口Ⅰ期愈合。随访期内6例患者疼痛症状消失。关节肿胀、假性交锁症状均消失。X线片显示移植骨软骨位置良好。结论自体软骨移植能缓解关节软骨损伤后出现疼痛、交锁症状，修复后的软骨为透明软骨。
Objective To investigate the effects of using arthroscopic osteochondral autografting for the treatmant of osteochondral defects of weight-bearing knee joint surfaces. Methods 7 cases were selected in osteochondral damages of the medial femoral condyle. 2 mobile chondral fragments, and 3 synovial plica were removed, 3 partial medial meniscectomies were performed using arthroscopic techniques. 3 - 5 plugs were harvested in the superior medial femoral condyle, and replaced in the same holes in the medial femoral condylar osteochondral damage for the treatment of full-thickness defects of weight-bearing joint surfaces. Results The would healing was well in 7 cases. The pain and swelling were alleviated in 6-month follow-up. Joint locking was disappeared. The planted plugs position were excellent in radiograph evaluation. Conclusions Arthroscopic osteochondral autografting can relieve the pain and locking, and arthroscopic osteochondral autografting allows the restoration of hyaline articular cartilage with zonal matching of the graft.
