摘要
目的研究角膜上皮在大鼠穿透角膜移植排斥反应中的作用。方法近交系F344和Wistar大鼠为研究对象,建立穿透角膜移植模型。实验动物随机分为6组,分别为保留供体上皮、去除供体上皮和保留受体上皮的同基因及异基因组。术后观察排斥发生时间,并行苏木精-伊红染色及VEGF、CD4、CD8、RT1B的免疫组织化学染色。结果同基因移植组均未发生排斥反应,去除供体上皮组VEGF表达较保留供受体上皮组增多(P<0.01)。异基因组均发生排斥反应,保留受体上皮组植片存活时间较其他两组延长(P<0.01);植片中CD4、CD8和RT1B抗原的表达亦较其他两组少(P<0.01)。结论上皮在诱导排斥反应发生中具有重要作用,保留受体上皮可以明显延长穿透角膜移植植片的存活时间。
Objective To study the role of epithelium in the corneal transplantation rejection and determine whether epithelium-deprived corneal allograft covered with syngenic epithelium can promote the survival of corneal allograft. Methods Ninety-six inbred rats received allogenic ( F344 to Wistar) or isogenic graft ( Wistar to Wistar). These rats were assigned to six groups randomly. A group received full-thickness isogenic corneal graft with donor epithelium. B group received epitheliumdeprived isogenic corneal graft without epithelium from donor. C group got the isogenic corneal graft of donor stroma covered with recipient epithelium. D group received full-thickness corneal allograft with epithelium. E group got donor' s epithelium-deprived corneal allograft without epithelium, and F group got corneal allograft with donor' s stroma covered by recipient epithelium. Slit lamp examination were performed to record the rejection index everyday until 4 weeks in post-operation. The rats were killed in different time points and the eyeballs were enucleated for histological observation including HE and immunohistochcmistry staining of VEGF, CD4, CD8 and RT1B. Results The corneal grafts of isogenic groups kept clear without rejection during the observation period. Compared with A and C groups,the neovascularization and inflammation reaction of B group were obvious,and the VEGF expression level at 3 and 14 days was significantly higher (P 〈0. 01 ). In D,E and F groups,rejection occurred at(7.7 ± 1.64) days, (7.82 ± 1.66) days and ( 16.33 ± 1.73 ) days respectively. The survival time of graft in F group was considerably longer than that of D and E groups (P 〈0.01 ) and the expressions of CD4, CD8, RT1B were less than those of the D and E groups at postoperative 10 days (P 〈0.01 ). Conclusion Donor' s epithelium can promote the rejection of corneal allograft. Coverage of recipient epithelium-deprived donor can obviously improve the survival of grafts.
