摘要
目的:探讨survivin和CD133在急性白血病(AL)患者骨髓液中的表达及其意义。方法:应用逆转录聚合酶链反应(RT-PCR)检测50例AL患者和10例正常对照者骨髓液中survivin、CD133 mRNA表达,并将结果与AL临床特征、血液学特点及疗效相比较。结果:(1)AL患者骨髓液中survivin表达和CD133表达的阳性率分别为78%和42%,均高于正常对照组(P<0.05),其表达及表达强度与年龄、性别、WBC计数、碱性磷酸酶、肝脾及淋巴结肿大均无明显关系(P>0.05),survivin与化疗疗效有关(P<0.05),CD133与分型有关(P<0.05)。且表达强度越高,完全缓解率愈低(P<0.05)。(2)Survivin和CD133在AL骨髓液中的表达呈正相关性(r=0.6429)。(3)Survivin、CD133 mRNA均阳性的AL患者完全缓解率明显低于两者均阴性的患者(P<0.05)。结论:(1)Survivin和CD133在AL患者骨髓液中表达上调,提示其可能在白血病的发生、发展中起重要作用。(2)Survivin表达与化疗疗效有关,CD133与AL分型有关。(3)Survivin、CD133 mRNA可做为预后不良的因素之一。
Objective To evaluate the expressions of survivin and CD133 in acute leukemias （AL） and their clinical significance. Methods Reverse transcriptase polymerase chain reaction （RT-PCR） was performed to detect the expressions of survivin and CD133 mRNA in 50 patients with AL and 10 healthy individuals. The find!ngs were compared with the clinical features, hematological characteristics, and therapeutic responses of AL. Results （ 1 ）The expressions of survivin and CD133 were higher in AL than in normal controls with a positive rate of 78% and 42%, respectively （P 〈 0.05）. The levels of expressions were not markedly related to age, gender, leukocyte count, AKP, lymphadenopathy, or hepatosplenomegaly （P 〉 0.05）. The survivin expression was related with the efficacy of chemotherapy （P 〈 0.05） and the CD133 expression was associated with the type of AL （P 〈 0.05）. （2）The expression of survivin was positively correlated with that of C D 133 in AL（r=0.642 9）. （3）The C R in the patients with both survivin and CD 133 positive was evidently lower than that in those with both negative （P 〈 0.05）. Conclusion （1） The upregnlation of the expressions of survivin and CD133 in the bone marrow of the patients with AL suggests that it may play an important role in the development and progress of AL. （2） The expression of survivin is related to the efficacy of chemotherapy and that of CD133 is to the type of AL. （3）The expressions of survivin and CD 133 mRNA in AL could be used as one of the indicators of poor prognosis.
