B型利钠肽在2型糖尿病舒张性心功能不全患者诊断中的临床应用 被引量:1

DIAGNOSTIC VALUE OF B TYPE NATRIURETIC PEPTIDE ON DIASTOLIC HEART FAILURE IN PATIENTS WITH 2 TYPE DIABETES MELLITUS

摘要 目的探讨血浆B型利钠肽(BNP)浓度在2型糖尿病(DM)舒张性心功能不全(DHF)诊断的临床价值。方法选择58例经心脏超声检测符合心脏单纯舒张功能障碍的DM患者和54例心功能正常的DM患者,并以55例健康查体者作为正常对照组。对心脏单纯舒张功能障碍的58例DM患者,按不同二尖瓣舒张早期血流速度与二尖瓣舒张晚期血流速度比值(E/A)分为二组,即DHF1级组:0.8≤E/A<1(n=29例);DHF2级组0.6≤E/A<0.8(n=29例)。采用酶联免疫吸附法(ELISA)测定血浆中BNP浓度。结果DHF组的BNP浓度与正常对照组、心功能正常组的血浆BNP浓度分别为(23.28±13.47)ng/L、(5.22±2.71)ng/L和(11.04±6.07)ng/L,三组间两两比较差异均有统计学意义(P<0.05);血浆BNP浓度随着DHF程度加重而逐渐提高(P<0.05)。以血浆BNP浓度14.5ng/L为界值,BNP判断糖尿病DHF的敏感度为79.3%,特异度为83.3%。结论BNP在DM患者舒张性心功能不全中具有较好的临床诊断价值。 Objective To analyze the clinical significance of plasma B- type natriuretic peptide (BNP) concentration on diagnosis of diastolic heart failure in patients with 2 type diabetes mellitus. Methods Plasma level of BNP was measured with enzyme linked immunosorbent assay (ELISA) and ratio of mitral annular early diastole to late diastole (E/A) in 55 normal controls and 112 patients with 2 type diabetes mellitus were computed. According to the results of ratio of E to A, the DHF patients were subdivided into DHF 1 group(n=29):0.8≤E/A〈1;DHF 2 group (n=29):0. 6≤E/A〈0.8. Results The blood concentrations of BNP were significantly increased in diabetic patients with diastolic heart failure, compared with those with normal diastolic function (P〈0.05). when the cut-off value of BNP was more than 14.5ng/L,in diagnosis of DHF with diabetics, the sensitivity in prognosis of diastolic heart failure was 79.3 %,and the specificity was 83.3%. Conclusion The plasma BNP concentration may be a useful marker for diagnosis of the diastolic heart failure of diabetic patients.
