摘要
本文作者试图对３０余年针刺麻醉的认识加以总结，并对其今后的发展提出讨论。（１）针刺麻醉（ＡＡ）的核心是针刺镇痛。（２）针刺可以减轻外科手术引起的疼痛，但不能达到完全无痛。（３）针刺合并药物麻醉，可使麻醉药用量减少４０％～５０％，从颅脑到腹部手术均如此。（４）针麻的优势主要是由于可使麻醉药用量减少，从而使生命指标波动减少，术后恢复加速，术后住院日期缩短；也由于针麻下手术可使病人与医师有更好的配合。（５）针麻的难点主要在于针刺的操作引起一些困难，这些困难已由于韩氏穴位神经刺激仪（ＨＡＮＳ）的出现而得到解决。（６）建议今后应用针刺与药物复合麻醉，称之为“针刺辅助麻醉”（ＡＡＡ）。它既可保留ＡＡ的主要优点，又可保证病人在完全无痛的情况下接受手术。（７）从医院管理的角度应制订一些新的规定，鼓励麻醉医师在有利于病人的前提下多应用ＡＡＡ。
In this article the author tried to summarize more than 30 years' practice of acupuncture anesthesia(AA) in China,and to comment on the future development of this medical technique.(1) Acupuncture-induced analgesia is the key element of acupuncture anesthesia.(2) Acupuncture is able to reduce the pain produced by nociceptive inputs from surgical operations,but is theoretically impossible to use acupuncture for a toeal abolishment of surgical pain.(3) Quantitatively,acupuncture is able to reduce the amount of anesthetic drug by 40～50% in many surgical operations including the cranial and abdominal operations.(4) The beneficial points of AA in combination with drug anesthesia depend mainly on the reduction of anesthetic usage, resulting in a smoothening of the fluctuation of vital parameters during the surgery, acceleration of the post-operative recovery and shortening of the period of postoperative hospitalization,and partly on the possibility of surgeon-patient interaction and cooperation during the surgical procedure.(5) The difficulties of performing classical acupuncture manipulations have been solved since the availability of the Han's Acupoint Nerve Stimulator(HANS).(6) It is suggested to use acupuncture in combination with anesthetics,and this technique could be termed acupuncture-assisted anesthesia(AAA) which keeps most of the strong points of AA and puts the patient in a absolutely pain-free status.(7) From the hospital management point of view,there should be new regulations to encourage the anesthesiologists to use AAA whenever it is considered beneficial to the patient.
