摘要
目的评价经胸部乳晕入路腔镜甲状腺切除术的临床应用价值。方法采用经胸部乳晕入路施行腔镜甲状腺切除术,其中单纯甲状腺结节摘除术8例,单侧甲状腺大部切除术6例,一侧甲状腺大部切除并对侧甲状腺结节摘除4例,单侧甲状腺腺叶切除术3例。结果21例全部成功,手术时间95～245min,平均(112.0±30.2)min,平均术中出血(65.8±19.5)mL,术后并发1例暂时性喉返神经损伤。结论经胸部乳晕入路腔镜甲状腺切除术具有极佳的美容效果,是安全和可行的。
[Objective] To evaluate the clinical applications of endoscopic thyroidectomy via breast approach. [Methods] Endoscopic thyroidectomy via breast approach was performed in 21 patients, including tumor dissection in 8 cases,one lobe subtotal thyroidectomy in 6 cases, one lobe subtotal thyroidectomy and another lobe tumor dissection in 4 cases, one lobectomy in 3 cases. [Results] All 21 patients underwent endoscopic thyroidectomy successfully without conversion to open thyroidectomy. The average time of operation was 112.0±30.2 min and the average intraoperative blood loss was 65.8±19.5 mL. We observed 1 case of transient recurrent laryngeal nerve palsy. [Conclusions] Endoscopic thyroidectomy via breast approach is safe and feasible, with excellent cosmetic results.
