摘要 目的探讨原发性高血压病对早期肾功能影响的临床意义。方法通过肾动脉造影、内生肌酐清除率排除肾性高血压,检测入选的原发性高血压病患者的肾小球滤过功能指标和肾小管功能指标。结果高血压1、2、3组尿β2-微球蛋白(β2-MG)肌/酐(Cr)均高于对照组(P<0.05),尿N-乙酰β-D-氨基葡萄糖苷酶(NAG)/Cr在高血压2、3组明显增高(P<0.01)。高血压1、2、3组尿渗透压均低于对照组(P<0.01)。在3h尿微量白蛋白检测中,高血压3组高于对照组(P<0.01)。结论原发性高血压病在对早期肾功能的影响中,肾小管近端重吸收功能、远端浓缩功能较肾小球滤过功能更易受损。 Objective To study early renal dysfunction in patients with essential hypertension, Methods Examined renal tubular function and glomerular filtration function in essential hypertension, after excluded renal hypertension by renal artery angiography and creatinine clearance. Results Urine beta2 - microglobulin was significantly different between patients and the control ( P 〈 0. 05 ) , urine N- acetyl- beta- glucosamindase in NO. 2 and 3 tested groups were obviously increased ( P 〈 0.01 ), as two corrected for serum creatinine. Urine osmolality was distinctly decreased in three tested groups ( P 〈 0. 01 ). 3 - hour urine microalbumin tended to be only slightly higher in NO. 3 tested group than in the normotensive control group ( P 〈 0.01 ). Conclusion Renal proximal tubular reabsorption function and dital tubular concentration function were more damaged than glomerular filtration function in essential hypertension.
