重症监护病房人工气道气囊压力连续监测的方法探究 被引量:51

摘要 目的连续监测重症监护病房有创呼吸机治疗的气管插管建立人工气道的危重病人气囊压力的变化。方法采用专用的PORTEX气囊压力监测表,通过首次气囊压力校正后,分别于校正后2 h、4 h、6 h、8 h连续监测患者气囊压力。结果首次气囊压力校正后2 h、4 h、6 h8、h,患者气囊压力分别为(2.85±0.23)kPa,(2.00±0.35)kPa,(1.81±1.98)kPa以及(1.77±0.00)kPa。校正后6 h参与气囊压力测量人数降为初始测量人数的50%,校正后8 h参与气囊压力测量人数则降为初始测量人数的12.5%。结论对于收入重症监护病房治疗的气管插管建立人工气道的危重病人,应该至少每隔4 h注气校正一次,才能防止套囊漏气及相应并发症的发生。 Objective To probe into the application values of pressure continuous monitoring of tracheal tube cuff pressure for nursing care of critical patients accepted tracheal catheterization. Method Patients in trial group were given PORTEX aerocyst pressure monitoring meter to aerify and to take pressure at 2h,4h,6h and 8h respectively after correcting for the first time. Result The aerocysts pressure of patients was 2.85±0.23 kPa,2. 00±0. 35 kPa, 1.81± 1.98 kPa,1. 77±0. 00 kPa at 2h,4h,6h and 8h respectively after correcting for the first time, The number of patients who participating measured declined to inchoate 50% at 6h. And it even declined to inchoate 12.5 % at 8h. Conclusion Critical patients accepted tracheal catheterization should be aerified and took pressure every 4h at least to keep the aerocyst pressure correct, then this could avoid either air leakage or tracheal mucous injury effectively.
