髓芯减压加自体多能干细胞、脱钙骨基质植入治疗早期股骨头坏死 被引量:25

摘要 目的:观察髓芯减压加自体多能干细胞、脱钙骨基质(DBM)植入治疗早期股骨头坏死的疗效。方法:采用髓芯减压加自体多能干细胞、DBM植入术治疗股骨头缺血性坏死80例(96髋),男43例,女37例;单髋64例,双髋16例;年龄21~62岁,平均38.4岁;病史8个月~3年,平均1.4年。按ARCO分期标准:Ⅰ期62髋,Ⅱ期34髋。根据术前术后症状、髋关节功能评分、X线及CT结果进行评价,分析其疗效。结果:随访超过2年的52例(61髋)中46髋效果明显,10髋病情缓解,5髋病情无进展,优良率为91.8%(56/61)。髋关节Harris评分由术前平均(49、1±7.5)分增至术后平均(75.2±5.9)分,差异有显著性统计学意义(P〈0.01)。结论:髓芯减压加自体多能干细胞、DBM植入术是治疗早期股骨头坏死的一种有效方法。 Objective: To study therapeutic results of core decompression combined with implantation of self-multipotent stem cell and decalcified bone matrix (DBM) for early femoral head necrosis. Methods: Among 80 patients with early femoral head necrosis (96 hips),43 patients were male and 37 patients were female, age ranging from 21 to 62 years( average of 38. d years). Unilateral head necrosis occurred in 64 hips and bilateral head necrosis occurred in 16 hips. The average medical history was 1.4 years ranging from 8 months to 3 years. According to standard of ARCO staging,62 hips were stage Ⅰ and 34 hips were stage Ⅱ. All the patients were treated with core decompression combined with implantation of self-multipotent stem cell and DBM. Clinical effect was analyzed according to clinical symptoms, the score of hip joint function, the films of X-ray and CT before and after operation. Results: Fifty-two patients (61 hips)were followed up more than 2 years. Forty-six hips obtained excellent results, 10 hips got relief of clinical symptom ,5 hips had no improvement, the rate of excellent and good was 91.8% ( 56/61 ). The averagely Harris score increased from preoperative 49. 1 ± 7.5 to postoperative 75.2 ± 5.9 (P 〈0. 01). Conclusion: Core decompression combined with implantation of self-multipotent stem cell and DBM is an effective method for early femoral head necrosis.
