摘要
目的探讨子宫动脉栓塞术治疗子宫肌瘤的疗效、安全性,以及并发症的防治。方法对51例子宫肌瘤患者采用赛丁格(Seldinger)技术,行双侧子宫动脉栓塞治疗。结果术后随访12个月。临床症状明显改善,月经周期无改变,经期缩短,月经量减少至术前的(48.6±15.3)%(P<0.01)。伴有贫血者术前血红蛋白值(Hb)为(80.2±17.8)g/L,术后3个月恢复至(123.5±14.3)g/L(P<0.01)。子宫及肌瘤体积呈进行性缩小,分别由术前的(356.1±102.2)cm3和(112.7±59.2)cm3缩小至术后6个月的(168.5±62.1)cm3和(42.4±13.2)cm3(P<0.05),术后1年缩小至(145.8±48.7)cm3和(35.6±10.2)cm3(P<0.01)。所有病例无严重并发症。结论子宫动脉栓塞术治疗子宫肌瘤安全、有效,能减轻症状、改善贫血,减少月经量,缩小瘤体。
Objective To assess the therapeutic efficacy, safety and side effects of uterine arterial embolization（UAE） on uterine myoma. Methods UAE was performed in 51 patients with uterine myoma by the Seldinger technique. All patients were followed 12 months to observe the changes of clinical symptoms and uterus and myomata size. Results All clinical symptoms of 51 patients were relieved. Catamenia shortened and menorrhagia reduced to （48.6 ±15.3） % （P 〈0. 01） with periods unchanged. Hemoglobin （Hb） retumed from （80.2 ± 17.8） g/L before operation to （ 123.5 ± 14. 3 ） g/L （ P 〈 0. 01 ） in patients with seeondary anemia. The size of uterus and myomata decreased from （356.1 ± 102.2） cm^3and （ 112.7 ±59.2）cm^3 before operation to （168.5 ±62. 1） cm^3 and （42.4 ± 13.2） cm^3 （P 〈0.05） after 6 months, and （145.8 ±48.7） cm^3and （35.6 ± 10. 2） cm^3 （P 〈 0. 01 ） after 1 year, respectively. In the course of the therapy, no severe complications appeared. Conclusion UAE is an .effective and safe therapies in uterine myoma. This method can lighten symptom, improve anemia, reduce menorrhagia and decrease the volume of utems and myomata.
