摘要
采用随机、双盲法对特发性突聋病例进行了微波辐射与单纯药物治疗效果的比较,从而对微波的疗效作出重新的评价。结果表明微波治疗组的有效率为8181%,听力达实用水平者为5454%;而药物治疗组的有效率为2727%,两组间有极显著差异(P<0001)。对发病后2周以上、2个月以内的患者,药物组中无1例听力有任何程度的恢复;而在微波组的6例中,有3例听力有不同程度的恢复,其中有2例竟恢复到实用水平以内。资料还表明年龄的大小对治疗的效果影响不明显,在50~75岁与7~49岁两年龄亚组的比较中,其间并无显著差异(P<005)。然而,在微波与药物治疗两组间相比,其间差异显著(P<001)。结果表明微波对特发性聋的疗效较药物治疗优越。作者在以前实验结果的基础上认为微波治聋的特点在于其能量集中,能穿透组织达一定深度。因此,对耳辐射时能在内耳发生直接的生物效应,其作用机制是改善内耳微循环的血流。
Using randomized,double blind trial,we compared the treatment with(a) medicine alone,(b) microwave irradiation in 33 patients with idiopathic sudden hearing loss; hence a reappraisal of microwave's effect was made. Results indicated in patients of microwave group, the total efficiency of hearing improvement was of 81 81%, in which a recovery of serviceable hearing was of 54 54%; however, the total efficiency of heaing improvement in patients of medicine group was of 27 27%, with a significant difference ( P <0 001) between the two groups. Considering the start of treatment between 2 weeks to 2 months after onset, none of medicine group showed any improvement of hearing, but of six cases in microwave group, 3 cases gained a hearing recovery in various degrees, among them 2 cases improved up to serviceable level. Also, it seems that age does not have influence on the effect of treatment. Comparing the results of our series,there was no difference ( P >0 05) in improvement of hearing between the 50~75 and 7~49 years old groups. However, comparison of the results between the microwave and medicine group,showed that a significant difference existed. Overall, the effect of microwave therapy may give an advantage over medicine alone. Based on our previous animal experiments, we considered that the characteristics of microwave therapy in its concentration of energy which could penetrate a certain depth into the irradiated zone; therefore, a direct bio effect could be produced in the inner ear. Both the improvement of cochlear blood flow and increase of the filling of red blood cells in stria vascularis were the action mechanism assumed.
