摘要
目的探讨胸腔镜辅助下小切口前路手术治疗陈旧性胸腰段脊柱骨折的疗效。方法对11例陈旧性胸腰段脊柱骨折患者行胸腔镜下脊髓减压、植骨、Z-Plate钢板内固定术。结果全部病例切口Ⅰ期愈合,平均随访10个月,术后椎体间均骨性愈合,大部分患者神经功能有不同程度恢复。并发肋间神经痛1例,胸腔积液1例,均治愈。结论该方法具有组织损伤小、视野清晰、骨折复位满意、恢复快和并发症少等优点。
[Objective] To discuss the results of anterior approach and small-incision for old thoracolumbar spinal fracture surgery assisted by thoracoscope. [Methods] Eleven cases of old thoracolumbar spinal fracture were treated with decompression of spinal cord,auto-iliac bonegraft and Z-Plate internal fixation assisted by thoracoscope. [Results] All incisions were primary healing.The average follow-up time was 10 months.All got intervertebral bony fusion.The function of spinal cord was improved in most patients.1 case with lesion of intercostal nerve,1 case with pleural effusion,both complications took a favorable turn. [Conclusion] It is observed that this technique had many advantages such as less tissue damage,clear operation vision,good reduction, rapid recovery and few operative complication.
