山莨菪碱对ARDS的氧输送及血流动力学改变的作用 被引量:2

Effects of anisodamine on oxygen transport and hemodynamics in patients with postoperative ARDS

摘要 早期、大剂量山莨菪碱治疗外科术后成人呼吸窘迫综合征(ARDS),观察病程第1~4天氧输送与血流动力学等指标。治疗组用山莨菪碱+常规治疗,对照组用常规治疗。结果:第2~4天治疗组CI、DO_2、VO_2、O_2Ext、PaO_2/FiO_2分别显著高于对照组(P<0.05);A-aDO_2、PVRI显著低于对照组(P<0.05)。VO_2与DO_2呈直线相关(r=0.58,P<0.05)。治疗组8例死亡1例,对照组7例死亡5例(0.01<P<0.025)。结论:术后ARDS早期大剂量山莨菪碱应用可维持高水平的CI和DO_2,改善肺循环、肺弥散功能及组织氧利用,降低死亡率。 Fifteen patients with postoperative ARDS were studied to investigate the effects of Anisodamine on oxygen delivery and hemodynamics during first 4 ARDS days. Eight of them (Ani group)were given continuous and agressive doses of anisodamine in early ARDS stage. The result showed that Ani group had significant higher levels of CO, DO_2, VO_2,O_2Ext, PaO_2(P<0.05)than those of the control group from second to fourth day. 1 of 8 cases of Ani group died of MOF and 5 of 7 cases of control group died of MOF (P<0.05). There was a correlation between DO_2, and VO_2 in this whole series(r=0.58). It is concluded that massive dose of anisodamine may be used in early ARDS stage to maintance high levels of CO and DO_2, and improve pulmonary circulation, diffusion function and oxygen utilization of tissue in patients with postoperative ARDS.
