新型液态栓塞剂Onyx栓塞枕大孔区动静脉瘘1例

摘要 病例患者，女，48岁。突发头痛、恶心、呕吐4h入院。查体：T36．4℃，P82次／min，R19次／min，Bp126／6mmHg。神志清楚，双瞳等大等圆，对光反射正常，脑膜刺激征阳性。急诊头颅CT示“蛛网膜下腔出血，并三、四脑室内积血”（图1）。给予脱水、降颅压、防治脑血管痉挛等药物治疗。1周后在全麻下行全脑血管造影显示：枕大孔区硬脑膜动静脉瘘，其供血动脉为左侧咽升动脉，引流至岩上窦（图1）。随即行介入栓塞术，经左侧入路将5F单弯导管选择性插管于左侧椎动脉开口处，再将6F导引导管经右侧入路置于左侧咽升动脉开口处，造影明确病变血管情况后，运用同轴导管技术，经导引导管在微导丝引导下，将Margic微导管超选择性插管于左侧咽升动脉并到达枕大孔区硬脑膜动静脉瘘口处，再次造影证实瘘口及引流静脉形态显影清晰。
