围CPB期血浆内源性洋地黄样活性物质的变化 被引量:2

Changes of plasma endogenous digitalis-like substance in human during cardiopulmonary bypass

摘要 观察15例病人围CPB期血浆内源性洋地黄样物质(EDLS)变化。结果:① EDLS麻醉后即下降,至主动脉开放前及心脏复跳后5分钟达最低值,心脏复跳后30分钟回升近麻醉前水平;② CVP、MAP与EDLS变化趋势一致,EDLS与CVP正相关;③主动脉阻断<45分钟、心脏自动复跳病人与主动脉阻断≥45分钟、心脏除颤病人比较,主动脉开放前、心脏复跳后5分钟和30分钟三个时点的EDLS均相应较低。提示:EDLS的降低对心脏自动复跳和减轻心肌损害具有积极作用。 The plasma level of endogenous digitalis-like substance(EDLS) was measured in 15 patients who underwent cardiac operation under cardiopulmonary bypass (CPB). Blood samples were taken before and after anesthesia induction,immediately before CPB and removing the aortic crossclamp,and at 5,30min after heart rebeating. Resluts showed that the plasma EDLS level slightely decreased after anethesia indction, and significantly decreased immediately before removing the aortic crossclamps and at 5 min after heart rebeating (P<0.05),and recovered close to baseline at 30 min after heart rebeating. Changes of EDLS levels were similar to those of CVP and MAP, in addidtion,there was a significant positive correlation between EDLS and CVP (r=0.78,P<0.05);in the patients with aortic crossclamping time less than 45 min and hearts spontanously being restored,the levels of EDLS were lower than ones in those with aortic crossclamping time more than 45 min and the hearts being defibrilated, immediately before removing the aortic crossclamp, and 5、30 min after heart reheating,respectively. These resuits suggest that plasma EDLS level is associated with the circulation function especially with the CVP,and its decrease benefits the spontanous restoration of beats.
