摘要
目的通过处方审核暴露问题,促进临床合理用药。方法每月随机抽取门急诊处方,根据说明书、临床药理学、新编药物学等专业书籍及文献资料,对不合理处方进行分析评议。结果不合理处方占不合理处方的4.20%;滥用抗生素的处方占不合理处方的47.62%。不合理用药主要表现在不合理的联合用药、重复用药、用法用量不当、溶媒选择不当、药理药效拮抗、毒性相加。结论临床药师的处方点评是直接调查分析不合理用药的重要手段,能有效地促进、指导临床合理用药。
Objective To promote safe, effective, economical and proper medication by checking prescription. Method Prescriptions in the outpatient and emergency departments of our hospital were randomly selected; the irrational prescriptions were analyzed with regard to drug descriptions, pharmacological books and literatures. Result The irrational one amounted to 4. 20% of the total selected prescriptions; antibiotics drug abuses amounted to 47.62% of the irrational ones. The irrational drug use problems mainly seen in unreasonable drug combination, repeated dosage, irrational dosiology and method of administration, menstruum incompatibility, pharmacodynamics conflict and combined toxicity. Conclusion Analyzing prescription of clinical pharmacist is an important pathway of di- rectly investigating irrational drug use. It can effectively promote and guide rational drug use.
