摘要
目的观察肠不完全性缺血再灌注(UR)损伤对远位免疫器官的影响。方法55只大耳白兔分为肠不完全性I/R组、假手术组、正常对照组。依据失血性休克过程中肠血流量变化规律,不完全性阻断肠系膜上动脉(SMA),复制肠不完全性I/R损伤模型,观察脾脏、肠系膜淋巴结(mesenteric lymphnode,MLN)细胞增殖力变化,及细胞凋亡和caspase-3的改变,并行静脉血、脾脏、MLN细菌培养。结果肠缺血期,脾脏、MLN凋亡细胞略增加,再灌注后,凋亡细胞增加明显,3d时仍见较多凋亡细胞;脾脏、MLN细胞caspase-3表达与凋亡改变趋势相同;肠不完全性I/R损伤后脾脏、MLN细胞增殖力明显降低;静脉血、脾脏、MLN细菌培养阳性率明显增加。结论脾脏、MLN等免疫细胞功能障碍可能是肠I/R损伤后细菌移位的重要原因,其发生发展与肠I/R损伤后caspase-3激活所致的细胞凋亡有关。
Objective To investigate the influence of partial isohemia and reperfusion (I/R) injury of intestine on spleen and mesenteric lymphnode (MLN). Method Fifty-five white rabbits were divided into 3 groups: partial I/R of intestine, sham operation, and normal control. In intestine I/R group, according to the changes of superior mesenteric artery (SMA) blood flow during hemorrhagic shock, the SMA was partially blocked, and the blood flow reduced to 50% for 4 hours. Stimulated with phytohem-agglutinin (PHA) for 24 hours, the preliferation capacity of spleen cells and MLN cells was measured, and the changes in the number of apoptotic cells and the level of caspase-3 cystainl asparate specific preteas in spleen cells and MLN cells were also observed. Meanwhile, the portal vein blood, spleen tissue and MLN were cultured to investigate the bacteria translocation. Results In isohemia stage, apoptotic cells in spleen and MLN increased slightly, and then increased dramatically after reperfusion stage, a large number of apoptotic cells still existed in spleen and MLN at 3 days after operatian. The expression of capase-3 in spleen and MLN showed the same changes as that of apoptotic cells. After I/R injury of intestine, the preliferatian capacity of spleen cells, and MLN cells was decreased dramatically, and the positive ratio of bacterial growth was high in the portal vein blood, spleen tissue and MLN. Conclusions The immune dysfunction of spleen and MLN maybe the key point of bacterial translocatian after I/R injury of intestine, which was associated with the apoptosis induced by activated capase-3.
