老年心脑血管疾病心肌灌注、左室射血分数与认知相关性及随访研究

摘要 目的了解老年缺血性心脑血管疾病患者心肌血流灌注及左室射血分数(LVEF)对认知功能的影响。方法对68例老年缺血性心脑血管疾病患者进行心肌血流灌注SPECT检查,同时应用MMSE量表、CISA量表和临床记忆量表检测认知功能。并于12个月后进行随访,动态监测心肌血流灌注和认知功能发展情况。结果心肌血流灌注与认知功能无明显相关,而LVEF与认知记忆能力,如记忆商、指向、联想及人像分测验成绩呈正相关(r=0.411,0.406,0.343,0.439,P<0.05)。随访研究发现总体样本和冠心病组的平均心肌灌注水平下降明显(P<0.05);CISA测验中多项得分(B因素、D因素、言语商、操作商、拼块、词义辨析、图画找错和背数)显著降低(P<0.05),分组后冠心病组和脑血管病组认知得分下降均明显。结论心功能的下降对老年缺血性心脑血管疾病患者的认知功能影响甚大。
