摘要
目的探讨凋亡调控因子Survivin和Caspase-3在子宫内膜癌发生发展中的作用及二者之间的关系。方法采用免疫组化SP法对13例正常子宫内膜、28例增生性子宫内膜及50例子宫内膜癌中Survivin和Caspase-3表达进行了研究。结果①正常子宫内膜、增生性子宫内膜、子宫内膜癌中Survivin蛋白表达阳性率分别为61.5%、85.7%、86.0%,三组间无显著性差异(P>0.05)。其过表达(-)阳性率分别为7.6%、14.3%、40.0%,子宫内膜癌与正常子宫内膜及增生性子宫内膜间均有显著性差异(P<0.05)。随着子宫内膜腺癌组织学分级的升高,Survivin过表达阳性率增加,G3、G2与G1间均有显著性差异(P<0.05)。②正常子宫内膜、增生性子宫内膜、子宫内膜癌中Caspase-3蛋白表达阳性率均为100%,各组间无显著性差异(P>0.05)。其过表达阳性率分别为92.3%、92.8%、60.0%,子宫内膜癌与正常子宫内膜及增生性子宫内膜间有显著性差异(P<0.05),在子宫内膜腺癌组织学分级G1、G2、G3中,Caspase-3过表达率呈下降趋势,但三组间无显著性差异(P>0.05)。③子宫内膜癌中Survivin过表达与Caspase-3表达呈负相关(rs=0.287,P<0.05)。结论子宫内膜癌中Survivin过表达增加,Caspase-3表达下降,机体抑制凋亡能力增强,促凋亡功能减退,打破了细胞增殖与凋亡之间的平衡,在子宫内膜癌发生中可能起一定作用,也可能与子宫内膜癌的恶性生物学行为有关。
Objective To investigate the functions of apoptosis modulator Survivin and Caspase-3 in developing endometrial carcinoma and the relationship among them. Methods We studied the expression of Survivin and Caspase-3 in 13 cases of normal endometrium, 28 cases of proliferative endometrium, and 50 cases of endometrial carcinoma with immunohistochemical SP methods. Results ① The positive rate of Survivin in normal endometrium, proliferative endometrium, and endometrial carcinoma was 61.5%, 85.7% and 86.0%, respectively, with no significant difference among those three groups (P〉0.05). The overexpression (++-+++) rate in the three group was 7.6%, 14.3% and 40.0%, respectively, and the difference among normal endometrium, proliferative endometrium and endometrial carcinoma were significant (P〈0.05). With endometrial adenocarcinomatous histological degree increasing, Survivin overexpression positive rate increased, and the differences among G3, G2 and G1 were significant (P 〈 0. 05). ② The positive rate of Caspase-3 in normal endometrium, proliferative endometrium, and endometrial carcinoma was all 100% with no significant difference among them (P〉0.05). However, the overexpression rate was 92.3%, 92.8%, and 60.0%, respectively. The differences among normal endometrium, proliferative endometrium and endometrial carcinoma were significant (P〈0.05). In different endometrial adenocarcinomatous histological degrees, G1, G2 and G3, the overexpression rate of Capase-3 showed the trend of decrease; however, there was no significant differences among the three groups (P〉0.05). ③The overexpression of Survivin and the expression of Caspase-3 in endometrial carcinoma showed negative correlation(rs=0.287, P〈0.05).Conclusion In endometrial carcinoma, the expression of suvivin increased, whereas the expression of Caspasc-3 dcereased. The ability to suppress apoprosis enhanced. whereas the ability to promote apoptosis reduced. The balance between proliferation and apoptosis was broken, which might be one of the causes of developing endometrial carcinoma and related to its biological malignant behavior.
