摘要
目的:探讨黏液样肾上腺皮质肿瘤的临床病理特点及其诊断和鉴别诊断要点。方法:行常规HE和超微结构观察．组织化学AB/PAS和免疫组化EnVision染色,检测2例黏液样肾上腺皮质肿瘤。结果:肿瘤呈实质性,切面有黏液感。肿瘤细胞在黏液背景中排列成相互吻合的网状或假腺腔样结构,腺腔内和细胞间为嗜酸性黏液。肿瘤细胞不同程度表达抑制素-α(inhibin-α)、黑素细胞相关蛋白A(melan-A)、波形蛋白(vimentin)、突触素(synapsin)和钙视网膜蛋白(calretinin)。超微结构示肿瘤细胞间有无定形黏液样物质,胞质内含丰富的内质网。结论:黏液样肾上腺肿瘤是罕见的肿瘤,组织化学、免疫组化和超微结构检测有助于肾上腺皮质黏液性肿瘤的诊断和鉴别诊断。
Objective To study the clinicopathologic characteristics and the differential diagnosis of myxoid adrenocortical neoplasm.Methods Two cases with myxoid adrenocortical neoplasm were studied by light microscope and immunohistochemical staining.Case 1 was also inverstigated with electorn microscope.Results Gross examination revealed solid and myxoid on the cut appearance.Microscopically,the tumors were characterized by variant proportion of myxoid components with a number of architectural pttems including delicate,anatomizing cords and a pseudoglandular pattem with distinct central lumens and intraluminal myxoid contents.The myxoid material stained positively with Alcian blue（pH2.5）.Immunohistochemical staining showed that the tumor cells were positive for inhibin-α,melan-A,vimentin,synapsin and ealretinin.Ultrastructural feature showed weel-developed,rough and smooth endoplasmic reticulum.Conclusions Myxoid adrenocortical neoplasm is a rare tumor in the adrenocortex.Histochemical,immunohistochemical staining and ultrastructural studies were useful in the diagnosis and differential diagnosis.
