血管束植入在组织工程骨血管化构建中的作用 被引量:17

摘要 目的观察以血管束植入法进行组织工程骨体内血管化构建的效果。方法将以生物活性陶瓷β-磷酸三钙为基质材料、以培养扩增和成骨诱导后的兔骼骨骨髓基质干细胞为种子细胞复合构建组织工程骨。将组织工程骨植入兔股骨干15mm缺损区,同期在显微镜下进行血管束解剖分离并植入其内。以未做血管束植入动物作为对照。4周时行活体探查,12周时测骨密度并取材行大体观察和组织学观察。结果植入血管处组织工程骨形成类似滋养孔的结构,血管双向通畅;植入血管束的组织工程骨受周围组织血管长入和血管束外生血管的双重营养。骨化方式材料表面为膜内成骨,材料内为多骨化中心的软骨内成骨。骨密度测量结果显示血管束植入后组织工程骨新骨生长较未植入血管束者多。结论血管束植入法可以加快组织工程骨在体内的生理性改建转化过程,是一种可行的组织工程骨血管化构建方法。 Objective To observe the effect of transformation of vascularized tissue-engineered hone with vascular bundle graft in vivo. Methods The hone marrow stem cells (BMSCs) obtained from rabbit os ilium were cultured, expanded and induced to form osteocytes, then they were combined with porous β-tricalcium phosphate (β-TCP) to construct tissue-engineered hone. The fissue-engineered hone was implanted in a segmental defect with critical length of 15mm in rabbit femoral shaft. A bundle containing both artery and vein was inserted in the centre of the tissue-engineered hone with microvascular surgical technique. After an examination with DEXA hone scanner, the specimen was harvested for macroscopic and histological examination after 12 weeks of growth period. Results The site where the implanted vascular bundle inserted into the tissue engineered hone appeared like foramen nutriens, the blood vessels were patent. Abundant blood vessels, which emerged from the deep tissue of the engineered hone, were evident on the surface of specimen. Multicentrie ossification area rich in blood vessels could be seen in the tissue-engineered hone histologically. The enehondral ossification in the center of tissueengineered hone and membranous ossification in the periphery occurred simultaneously. Some materials in the centre of tissuengineered hone had transformed into marrow cavity like construction. The results of the DEXA demonstrated that the vascularized tissue-engineered hone produced more honey tissue. Conclusions Implantation of blood vessel bundle into tissue-engineered hone may enhance neovascularization of the tissue-engineered hone and accelerate the process of reconstruction subsequently, and it is a promising method of vascularization of tissue-engineered hone.
