彩色多普勒检测早产儿餐前后肠系膜上动脉血流改变与喂养耐受性的研究 被引量:6

摘要 目的探讨餐前后早产儿肠系膜上动脉(SMA)血流变化与喂养耐受性的关系,以期通过胃肠血流动力学改变来预测是否发生喂养不耐受。方法彩色多普勒超声测定40例早产儿第一次喂养及足量喂养[100kal/(kg·d)]餐前餐后SMA血流参数变化,并记录有无发生喂养不耐受及达足量喂养的天数。结果①第一次喂养,不耐受组餐前餐后血流无明显变化(P>0.05),耐受组血流有明显变化(P<0.05)。足量喂养时,耐受组与不耐受组餐前餐后血流均有明显变化(P<0.05)。②第一次喂养餐后Vtam的增加与达足量喂养天数呈负相关关系(r=-0.337,P<0.05)。结论喂养可使SMA血流动力学发生变化;可根据第一次喂养餐前餐后SMA血流动力学改变(特别是Vtam的增加)来预测喂养风险,从而帮助调整早产儿喂养方案。 Objective To study the relationship of changes of blood flow velocity in superior mesenteric artery (SMA) before and after the feeding with color Doppler ultrasonography and tolerance to enteral feeding in premature neonates. Methods With the use of Doppler color ultrasonography, blood flow velocity of SMA was measured before and after the first feeding and full enteral feeding in 40 preterm infants. Both appearing feeding intolerance and the number of days of reaching adequacy enteral feeding was recorded. Results (1)By the first feeding, there were no significant difference of blood flow velocity before and after the feeding in the intolerant group (P〉0.05), but significant difference in the tolerant group (P〈0. 05). (2)There was a significantly negative relation (r=-0. 337, P〈0. 05) between the increase of Vtam in the SMA after the first feeding and the number of days of reaching full enteral feeding. Conclusion Feeding could change blood flow velocity of the SMA; The clinicians could predict the risk of feeding in preterm infants on the basis of the changes of blood flow velocity in the SMA before and after the first feeding (specially the increasing of Vtam) and adjust the feeding program.
