摘要
目的：采用三维适形放疗（3DCRT）治疗非小细胞肺癌的临床近期疗效，探讨适形放疗结合其它治疗的最佳方式。方法：根据非小细胞肺癌肿瘤的大小、分期，用三维适形放疗联合其它治疗对32例病人进行治疗，并设同期对照组32例，用化疗加常规放射治疗，以对比研究。结果：32例治疗组病人中CR8例（25％），PR21例（65．6％），NR3例（9．4％），CR＋PR29例（90．6％），急性放射性食管炎7例（14．2％），急性放射性肺炎4例（12．5％），均为1～2级，治疗后KPS评分提高者31例，1年生存率为86％，中位生存期16．5个月。用化疗加常规放射治疗的32例非小细胞肺癌病人，CR＋PR21例为65．6％，中位生存期12个月，放射性肺炎及食管炎发生率显著增多。结论：在循证医学原则的指导下，三维适形放疗与其它治疗结合，取得较好的临床近期效果，明显改善了病人生存质量，毒副作用轻，病人可耐受。
Objective： To explore the therapeutic value of 3DCRT in the treatment of non small cell lung cancer （NSCLC） . Methods： According to the different grades and characteristic of organism of NSCLC, we treated 32 NSCLC patients by 3DCRT alone and combining with the other treatments, 32 patients with inoperatable non small cell lung cancer were treated by chemotherapy and conventional radiotherapy. Results： In therapy group , CR： 8 patients （25%）, PR：21 patients （65.6%） ,NR：3 patients（9.4% ）, CR ＋ PR：29 patients（90.6% ） ,the acute radiation esophagitis rate 21.8% （n =7）, the acute radiation pneumonitis rate 12.5% （n =4）, they were 1 -2 grade,the KPS of 31 patients were improved, 1 - year effective survival rate 86% , and the median survival period 16. 5 months. In the control group ,32 NSCLC patients were treated by chemotherapy and conventional radiotherapy, CR ＋ PR：21 patients（ 65.6% ）, the median survival period 12 months. Conclusion： Under the evidence- based medicine, we could combine the 3 DCRT with the other therapy methods to, get the better results, and improve the patient survival quality. The adverse reaction was small, and the patients were tolerant.
