摘要
目的研究在呼吸机所致肺损伤(VILI)的炎症反应中热休克蛋白(HSP70)表达的意义及其与细胞因子白细胞介素1β(IL-1β)、白细胞介素6(IL-6)的关系。方法健康成年新西兰白兔30只随机分为3组,致伤通气+3cmH2OPEEP(PEEP组),致伤通气+0cmH2OPEEP(ZEEP组),对照组(NVC),始终以正常条件通气。机械通气开始后4h处死动物。采用放射性免疫分析(RIA)方法测定血中细胞因子IL-1β、IL-6的含量。采用Western-blot法检测家兔肺组织HSP70的表达。结果家兔在致伤械通气4h后,血中IL-1β、IL-6的水平明显升高,肺组织HSP70的表达显著增加,且IL-1β、IL-6均与HSP70呈负相关,而在致伤械通气的同时加用PEEP,血清中IL-1β、IL-6的含量明显减少。结论HSP70可通过减少细胞因子的产生,保护肺组织避免VILI,而在机械通气过程中加用PEEP可保护肺组织减轻损伤。
[Objective] To investigate heat shock protein 70 (HSP70) expression and relation with interleukinl1β (IL-1β) and interleukin6 (IL-6) in a model of ventilator-induced lung injury (VILI). [Methods] Thirty adult rabbits were divided into three groups at random: injured ventilation +3 cm H20 PEEP, injured ventilation + no PEEP (ZEEP) and control group. All animals were subsequently ventilated for 4 hours. After 4 hours, rabbits were killed. IL-1β and IL-6 levels in blood were assayed using RIA methods. Expression of HSP70 were detected using West- era-blot methods. [Results] After injured ventilation for 4 hours, The levels of IL-1β and IL-6 were increased distinctly, and there was a significant higher expression of HSP70. In the ZEEP group high HSP70 levels were correlated inversely with IL-1β and IL-6 levels. There was significant lower levels of IL-1β and IL-6 in the blood of the mechanical ventilation with PEEP group. [Conclusion] We propose that HSP70 expression protects the lung against ventilator-induced lung injury by decreasing levels of cytokines, and mechanical ventilation with PEEP can protect the lung against ventilator-induced lung injury.
