摘要
目的探讨Bowen病的治疗方法。方法回顾性分析1978年9月至2005年8月收治的13例Bowen患者的临床资料。结果单发12例,多发1例;11例患者为包块切除术后病理检查偶然诊断为Bowen病;7例行扩大切除术,3例行扩大切除术+植皮术,1例行扩大切除术+放疗,2例行单纯放疗;8例患者再次手术后发现病变残存。结论Bowen病的手术中,切缘需距病灶5mm,并切除病变基底下方的皮下脂肪组织。对于不宜手术的病例可采用放疗和5-Fu软膏涂布等其它措施。
Objective To investigate the principles of diagnosis and treatment of Bowen＇s disease.Methods A retrospective analysis was performed in 13 cases of Bowen＇s disease admitted between October 1978 and August 2005.Results Single lesion in 12 patients and multiple lesions in one patient was found. 11 patients was diagnosed by accident after biopsy. 7 patients received extended operation,and 3 patients had extended operation and skin grafting,and 1 patient was treated by extended operation and radiotherapy,and 2 patients cured by radiotherapy. Re-operation found that 8 patients had residual carcinoma.Conclusion A five-ram surgical margin is advisable with a depth to subcutaneous fat for removal of a possible underlying carcinoma. For patient with lesion not suitable operation could be treated with other measure,including radiotherapy and topical agents such as 5-fluorouraeil.
