摘要 目的探讨慢性高原病(CMS)患者肝功能及血清酶的变化。方法选择移居海拔4300m3个月以上的汉族男性青年323名,其中CMS患者87名.为CMS组,其余236名为非CMS组。清晨抽取空腹静脉血检测血清总胆红素(STB)、直接胆红素(SDB)、总蛋白(TP)、清蛋白(BLA)、丙氨酸氨基转换酶(ALT)、门冬氨酸氨基转换酶(AST)、γ-符氨酰转移酶(γ-GT)、碱性磷酸酶(ALP)。以血红蛋白(18g〈血红蛋白〈21g,记0分,血红蛋白≥21g,记3分)及动脉血氧饱和度(动脉血氧饱和度≤85%,记3分)记分。分别以1,2,3分表示轻、中、重度。无CMS总记分为0—5分,轻度为6~10分。中度为11~14分,黄度为15分或更高。结果CMS组较非CMS组STB,AST,ALT,γ—GT增高,有显著性差异(P〈0.05);TP,BLA降低,有非常显著性差异(P〈0.01),SDB,ALP无统计学差异(P〉0.05)。结论CMS引起肝功能指标及血清酶活性异常,提示CMS伴有多种器官或组织受损。 Objective To explore the liver function and the serum enzyme changes of chronic mountain sickness (CMS) sufferers. Methods A total of 323 youth who stayed at a high altitude of 4,300 meters for more than three months were inspected for serum total biliruhin (STB), serum direct bilirubin (SDB) , total protein (TP) , albumin (ALB), alanine aminotransferase (ALT), aspartate aminotransferase (AST) , γ-glutamyl transferase (γ-GT) and alkalinephosphatase ( ALP ). Count points on hemoglobin (Hb) and oxygen saturation (SaO2 ) were recorded ( 18g 〈 Hb 〈 21g, getting 0 ponit; Hb ≥ 21 g, getting 3 points) (SaO2 ≤ 85% , getting 3 points). The patients with a total point amounting to 0 - 5 were considered as non-CMS, and those with a total point more than 5 were considered as CMS. Of the patients, 87 were diagnosed as CMS ( CMS group) and the others were diagnosed as non-CMS ( non-CMS group). Results STB, AST, ALT and γ-GT in the CMS group increased when compared with that in the non-CMS group, and there was significant difference ( P 〈 0.05 ). TP and ALB increased significantly in the CMS group than in the non-CMS group ( P 〈 0.01 ). There was no significant difference in SDB or ALP between the two groups ( P 〉 0.05 ) Conclusion CMS leads to the changes in liver function and serum enzyme activity, which indicates that multiple organs and tissue are damaged.
