实时三维超声心动图定量评价高血压患者左室功能及室壁运动同步性的初步研究 被引量:3

Quantification of Left Ventricular Function and Systolic Synchronicity Using Real-time Three-dimensional Echocardiography in Patients with Hypertension

摘要 目的利用实时三维超声心动图(RT-3DE)容积-时间曲线评价高血压患者左室整体收缩与舒张功能及室壁运动同步性曲线变化。方法高血压患者48例,20例健康对照者,根据左室心肌质量指数(LVMI)将高血压病分为无左室肥厚组(NLVH组)及左室肥厚组(LVH组)。实时三维超声心动图全容积(FullVolume)采集图像,进行容积-时间曲线分析,记录左室舒张末容积(EDV)、左室收缩末容积(ESV)、左室射血分数(LVEF)、左室最大射血速率(PER)、左室最大充盈速率(PFR)、左室16节段至最低收缩末容量时间标准差与最大差异(Tmsv16-SD、Tmsv16-Dif),并比较高血压组(NLVH组、LVH组)与健康对照组之间的差异。结果与正常对照组相比,高血压患者组与健康对照组的舒张充盈参数PFR降低较明显(P<0.05),NLVH组与LVH组间差异无统计学意义(P>0.05),其余参数之间差异无显著性意义。结论实时三维超声心动图容积-时间曲线参数可以评估早期左室舒张功能减低,以PFR较为准确,为临床左室舒张功能的评价提供了一个无创、客观的新方法,Tmsv16-SD、Tmsv16-Dif可用来评价整体及局部室壁运动同步性。 Objective To quantitatively assess left ventricular function and systolic synchronicity in hypertensive patients using volume-time curves on real-time three-dimensional echocardiography. Methods Forty-eight patients with hypertension and twenty normal subjects were enrolled. Philips iE-33 with X3- 1 probe was used to perform full volume real-time three-dimensional echocardiography, and the Global and 17- segmental volume curves were obtained by Q 1ab software. The end diastolic volume (EDV), end systolic volume (ESV), left ventrieular ejection fraction (LVEF), left ventrieular peak ejection rate (PER), left ventrieular peak flow rate (PFR), the time of minimal systolic volume of 16-segmental' standard deviation (Tmsv16-SD), difference (Tmsv16-Dif) were derived. Results There were significantly differences of PFR between the two groups (P〈 0.05), but no difference was present between NLVH and LVH (P〉0. 05) . Other parameters had no differences between the two groups (P〉 0. 05) . Conclusions Real-time three-eehoeardiographie volume-time curves can evaluate left ventrieular diastolic dysfunetionin in early hypertension stage. Tmsv16-SD and Tmsv16-Dif were potential parameters for assessing left ventricular systolic synchronicity.
