摘要
目的研究瓣膜置换病人围术期的心肌损害程度及米力农对其的影响。方法20例心功能Ⅱ级～Ⅲ级瓣膜置换术病人，随机分为米力农组和对照组，主动脉开放即刻米力农组10min内用负荷剂量米力农50μg／kg、继以0．5μg／（kg·min）持续静脉输注，对照组则只给予生理盐水。分别于麻醉诱导后开胸手术前（T0）、主动脉开放后30min（T1）、6h（T2）、24h（T3）取中心静脉血，免疫荧光法检测血清肌钙蛋白Ⅰ（cTnⅠ）。结果两组T1、T2、T3cTnⅠ均明显升高，米力农组cTnⅠ在T1、T2、T3显著低于对照组（P〈0．05）。结论体外循环下瓣膜置换术应用米力农能够有效减轻心肌损伤程度。
Objective To investigate the myocardial injury and the influence of milrinone on Treponin I in the perioperative cardiac valve replacement. Methods Twenty patients （pts） with heart function of NYttA class Ⅱ or Ⅲ, scheduled for cardiac valve replacement were randomly divided into 2 groups： Milrinone group and control group. Milrinone 50 μg/kg was given in i0 min immediately after unclamping of aortic and followed by a continuous infusion of 0.5 μg/（kg · min） during operation in milrinone group for 24 h after operation. The ,same volume of saline was administrated in Control group. The venous blood samples were obtained from the central vein at the following points： before operation （To） ; 30 rain after unclamping of aorta （T1） 6 h （T2） and 24 h （T3） after unclamping of aorta. Results Compared with that at To, the levels of CTn Ⅰ increased significantly from T1 to T3 （ P 〈 0.05） in both groups. Compared with that in control group, CTn Ⅰ level increased obviously （P 〈 0.05） in milrinone group from T1 to T3. Conclusion During cardiac valve replacement, milrinone was effective for myocardial preservation.
