摘要
目的:探讨肝血管平滑肌脂肪瘤的临床病理特点、诊断及鉴别诊断。方法:对2例肝血管平滑肌脂肪瘤进行光镜及免疫组化观察,并复习文献。结果:肝血管平滑肌脂肪瘤由扭曲的血管、平滑肌及脂肪组织构成。免疫组化:梭形、上皮样平滑肌细胞HMB45、ACTIN、Desmin均阳性,血管内皮细胞CD34阳性,成熟的脂肪细胞S-100阳性。结论:肝血管平滑肌脂肪瘤是一种少见的良性间叶组织肿瘤,由于其组织形态变化范围大,易造成误诊,上皮样平滑肌细胞HMB45表达呈阳性在诊断中起着相当重要的作用。
Objective： To analyze clinicopathological characteristics, diagnosis and differential diagnosis of the hepatic angioleiomyolipoma （HAML）. Methods： Two cases with the HAML were studied using light microscopy and immunohistochemistry, in combination with a review of literatures. Results： The tumors were composed of various amounts of three components： i.e. blood vessels, smooth muscle cells and adipose cells. Immunohistochemically, the smooth muscle cells were positive for HMB45, Desmin, Actin, the endothelial cells were positive for CD34 and the adipose cells were positive for S- 100. Conclusion： A correct diagnosis of HAML might be difficult due to various growth patterns and cell types. Positiveness of the HMB45 in the smooth muscle cells is a key feature for HAML.
