2型糖尿病高尿酸血症与高血压肾病的相关性分析

A study on the relationship of hyperuricemia with diabetic hypertention and diebetic nephropathy

摘要 目的通过检测2型糖尿病(DM)患者血尿酸水平,探讨高尿酸血症与2型DM合并高血压、肾病的关系。方法选择98例2型DM患者,分成2型DM无合并症(单纯DM)组、2型DM合并高血压(DMEH)组和2型DM合并肾病(DN)组,测定血尿酸(UA)、肌酐(Cr)、尿素氮(BUN)、空腹血糖(FBG)、糖化血红蛋白(HbAlc)、空腹胰岛素(FINS)和血脂水平。结果DMEH组和DN组血UA水平显著高于健康对照组,P<0.05或P<0.01;DN组血Cr水平明显高于健康对照组和单纯DM组,P<0.001或P<0.01;多元回归分析表明,2型DM患者血UA与DBP、甘油三酯(TG)、Cr呈显著正相关而与高密度脂蛋白-胆固醇呈显著负相关;血UA与FINS呈正相关。结论糖尿病高尿酸血症可能参与了2型DM高血压及肾病的发生、发展过程,对其检测可作为2型DM并发症的预告因子。 Objective To discuss the relationship among hyperuricemia, diabetes-combined high blood pressure and nephropathy,by detecting the level of the blood uric acid (UA)in the patients with type 2 diabetes (T2DM). Methods Ninety-eight cases of T2DM were collected, and were divided into 3 groups: the one with non-combined-disease(only with diabetic syndrome), the one with combined high blood pressure(DMEH), the one with combined nephropathy(DN), the level of blood uric acid (UA), creatinin(Cr), blood urea nitrogen (BUN), free blood sugar(FBG), saccharified hemoglobin (HbALc), free insulin (FINS) and blood fat was measured respectively. Results The level of UA in all experimental groups was significantly higher than that in contrast one, P values were respectively:P 〈 0.05,P 〈 0.01 ,P 〈 0.05; the level of Cr in DN group was significantly higher than that in contrast one and T2DM one, P values were respectively: P 〈 0.001 and P 〈 0.01; multiple regression analysis showed that UA in the T2DM patients was positively correlated with DBP, TG and Cr significantly, while it was negatively correlated with HDL-C; UA was positively correlated with FINS. Conclusion Diabetic high blood uric acid disease might affect the process of occurrence and development of high blood pressure and nephropathy. The detection of them could be a premonitory of T2DM-combined disease.
