风湿性心脏病二尖瓣狭窄患者血浆肾素-血管紧张素-醛固酮系统活性变化 被引量:2

Changes of the activity of circulating renin-angiotensin-aldosterone system in patients with rheumatic mitral stenosis

摘要 目的研究风湿性心脏病(风心病)二尖瓣狭窄(MS)患者血浆肾素-血管紧张素-醛固酮系统(RAAS)活性变化,并探讨其临床意义。方法MS患者55例,按有无心房颤动(房颤)分为两组:窦性心律组(SR组)25例,房颤组(AF组)30例;正常对照组(NC组)17例。应用放射免疫方法(RIA)测定清晨空腹卧位循环血浆肾素活性(PRA)、血管紧张素Ⅱ(AngⅡ)、醛固酮(Ald)水平。结果AF组、SR组患者循环PRA、AngⅡ、Ald浓度与NC组相比均显著增高(P<0.01),AF组与SR组比较,循环PRA、AngⅡ、Ald浓度亦均明显增高(P<0.05或<0.01)。结论MS患者存在循环RAAS的激活,房颤时激活更明显。 Objective To study the changes of activity of circulating renin - angiotensin - aldosterone (RAAS) in rheumatic mitral stenosis patients with and without atria fibrillation and discuss their clinical significance. Methods There were 55 patients with rheumatic mitral stenosis enrolled. According to patients with and without atria fibrillation, they were divided into two groups: sinus rhythm group ( SR group, 25 cases) and atria fibrillation group ( AF group, 30 cases). Seventeen normal subjects were selected as normal control (NC) group. The plasma concentration of renin, angiotensin (Ang) Ⅱ and aldosterone(Ald) were measured by the method of radioimmunoassay(RIA). Results The plasma level of plasma renin activity (PRA), Ang Ⅱ and Ald in AF group and SR group were significantly higher than those in NC group ( P 〈 0.01 ), and compared with SR group, the plasma level of those in AF group were also significantly increased ( P 〈 0.05 ). Conclusion Patients with rheumatic mitral stenosis is characterized by the activation of circulating RAAS,and its activation becomes more stronger during atrial fibrillation.
