摘要
目的探讨改良拦截一劈裂技术在硬核白内障超声乳化术中的应用效果。方法对89例(99眼)核Ⅲ级以上的白内障实施改良拦截一劈裂技术,即在核中心刻碗后将乳化针头嵌入核内深达1/2~3/4核厚度,利用负压的作用固定晶状体核,用劈核器绕经核的3点钟位赤道部进入核下,使劈核器头与乳化头相对应,二者共同用力挤压,将核碎成2块,最终碎成6~8块,并逐一乳化吸除。配合粘弹剂充填前房下的连续环形撕囊、水分离技术等,平均超声乳化时间1_7分钟。结果术中改良拦截一劈裂组的超声时间和有效超声时间均短于拦截一劈裂组,差异有统计学意义(尸〈O.05),术后7天视力≥O.5者40眼(74.07%);≥1.0者12眼(22.22%);拦截一劈裂组分别为23眼(51.11%)及5眼(11.11%)。主要并发症为后囊破裂12眼,角膜水肿33眼,后发障3眼。结论改良拦截一劈裂技术是硬核白内障超声乳化术中有效地碎核方法之一,具有超声时间短、并发症少、劈核更加容易、安全等特点,其操作简便、安全、有效,术后效果好。
Objective To investigate the effect of the modified stop and chop method on phacoemulsification of hard nucleus cataract. Methods The modified stop and chop or conventional stop and chop technique was performed on 54 and 45 eyes of hard nucleus cataract. Results The intraoperative US and EP were less in the modified stop and chop group than the conventional stop and chop group. The significant difference existed between the two groups (P〈0.05). At 3 days, 7 days, and 30 days postoperatively, the visual acuity of 0. 5 or better with correction was obtained respectively in 21 eyes, 40 eyes, and 47 eyes in the modified stop and chop group and 9 eyes,23 eyes and 36 eyes in the conventional stop and chop group. The major complications were corneal edema and posterior capsular rupture. Conclusion The modified stop and chop technique for hard nucleus cataract is favored because of its effectiveness, safety, easy manipulation, and less time-consuming and fewer complications.
