摘要
目的探讨乏氧显像剂99mTc-HL91肺显像对肺鳞癌诊断的临床价值。方法使用SPECT对32例肺癌和23例非肺癌患者静脉注射99mTc-HL91925MBq后行1、6h胸部平面显像及4h断层+CT融合显像。利用计算机感兴趣区(ROI)技术计算1、6h靶/非靶放射性计数比值(T/NT),结果32例肺癌1、6h平面显像T/NT比值分别为1·185±0·057及1·435±0·080,配对T检验,两组资料之间的差别有显著性(t=20·299,P<0·01);对照组23例1、6h平面显像双肺中部对应感兴趣区L/R分别为0·978±0·055和0·980±0·030。肺癌组与对照组资料1h与1hT/NT值比较,差异无显著性(F=0·039,t=13·175,P>0·05);6h与6hT/NT值比较,差异有显著性(F=26·349,t=26·035,P<0·01)。32例肺癌患者4h断层显像T/NT比值为4·735±1·337。与1h、6hT/NT分别比较,其差异具有显著性,(t值分别为14·811、13·607P均<0·01)。结论1、6h平面显像T/NT比值分别为随时间增加而增加,静脉注射4~6h肿块显影清晰。肺癌能大量摄取99mTc-HL91,癌组织中存在乏氧细胞。
Objective To evaluate the efficacy of ^99mTc-HL91 fusion imaging in diagnosis of pulmonary carcinoma. Methotis 32 patients with pulmonary carcinoma and 23 patients with non - pulmonary carcinoma accepted hypoxic seintigraphy of ^99mTc-HL91 one, four and six hours after injecting ^99mTc-HL91. Fusion imaging was acquired 4 hours after injecting radionuclide. T/ NT ratio also were obtained one, four and six hours after injecting ^99mTc-HL91. Results In pulmonary carcinoma group, the radioactivity was accumulated in all lung metastases before radiotherapy. The T/NT ratio was 1. 185 ± 0,057, 1.435 ± 0.080 in 1 and 6 hour with ^99mTc-HL91 plane imaging. The T/NT ratio had statistical difference one and 6 hours after injecting ^99mTc-HL91 plane imaging, In non - pulmonary carcinoma group, the L/R ROI radioactivity ratio was 0.978 ± 0,055,0.980 ± 0.030 in 1 and 6 hour with ^99mTc-HL91 plane imaging, In pulmonary carcinoma group, it was more accumulated tomography in 4 hour than plane imaging in 1 and/or 6 hour in the radioactivity of lung metastases before radiotherapy, The T/NT ratio was 4.735 ± 1.337, The T/NT ratio had statistical difference in 4 hour tomography and 1 hour or 6 hour plane imaging after injecting ^99mTc-HL91, 12 patients with pulmonary carcinoma before and after radiotherapy accepted hypoxic scintigraphy of ^99mTc-HL91 one, four and 6 hour after injecting 99mTc - HLgl. The T/NT ratio was 1.192 ± 0.065, 1.436 ± 0.082 before radiotherapy as well as 1.05 ± 0.13, 1, 07 ± 0.17 after radiotherapy in 1 and 6 hour with ^99mTc-HL91 plane imaging. It was 4.446 ± 1.337 and 2,01 ± 0.78 with tomography 4 hour before and after radiotherapy, showing significant difference ( P 〈 0.01 ). Conclusion The intensity of lesion in lung cancer patients tends to increase over a period of time. The T/NT ratio is higher in 6 hour than in one hour plane imaging after injecting ^99mTc-HL91 before radiotherapy in lung cancer. The lesion of lung cancer increases ^99mTc-HL91 uptake, There are hypoxic cells in lung cancer. The ^99mTc-HL91 uptake in lung metastases decreases after radiotherapy.
