摘要
目的利用汽枪枪弹制作复合性兔眼球破裂伤模型,观察眼破裂伤后视网膜早期临床表现、病理变化以及光感受细胞损伤机制。方法48只家兔随机分为无枪伤对照组,伤后1、3、6、12、24h组,共6组,利用汽枪枪弹制作弹伤性兔眼球破裂伤模型,光镜观察早期视网膜结构改变及视网膜感光细胞凋亡。结果伤后1、3h时视网膜感光细胞形态上轻度改变,6、12h时视网膜感光细胞明显改变,24h时视网膜各层结构紊乱。细胞凋亡6h在神经节细胞和内、外核层有少量表达,12~24h染色阳性的细胞明显增多。经方差齐性检验及经单因素方差分析。对照组与伤后1h组无显著性差异(P=0.062),与伤后3h组具有显著性差异(P=0.005)、与伤后6、12、24h组有显著性差异(P=0.000)。凋亡指数统计结果与细胞凋亡结果基本相同。结论眼球破裂伤病情严重、伤情复杂,早期视网膜损伤较重,6~12h时即发生不可逆改变。早期眼球破裂伤视网膜感光细胞可能有很多原因,而细胞凋亡是早期视网膜感光细胞损伤的重要因素。
Objective By using air gun bullet to make wound on rabbit eyeball to observe early clinical manifestation, pathology changes, and injury mechanism of the eyeball, so as to supply experimental basis for clinical treatment. Methods 48 white New Zealand rabbits were selected, which were randomly divided into 6 groups: later lh,3h,6h,12h and 24h after damaging, as wound groups; and control groups without wound. Wound model was made by a plastic gun to produce ruptured the model at the frontal eyeball. The early retinopathy and apoptosis of retinal cells were observed by light microscopes. Results The pbotoreceptor cells of retina had a little change between 1-3 hours, other changes like these obviously appeared between 6-12 hours. While 24 hours the retina became thin and its structure was disordered. The apoptosis had lower expression in ganglion cells and the inner and outer nuclear layer 6 hours latter, and obvious increasing of dye positive cells was observed between 12 and 24 hours. In the experiment: when checked by One-way ANOVA, there was no significant difference between the control group and 1 hour wound group was not significant (P=0.062), compared with 3 hours wound group, the difference was significant, and the comparison with 6,12,24 hours group was quite significant (P=0.000). When compared the statistics results of the apoptosis index, there was the same result as apoptosis of pbotoreceptors cell. Conclusion Traumatic eyeball rupture is very serious, the retina damage appeared obvious 6 hours latter when checked by light microscopes. Between 6 and 12 hours the damage became unrecoverable. The injury is caused by many factors, in which the apoptosis of the photoreceptor cells is an important mechanism.
